What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

The inspectors’ words, 344 words
“PE-SAFE/FUNCTIONAL/COMFORTABLE ENVIRONMENT CFR(s): 494.60 The dialysis facility must be designed, constructed, equipped, and maintained to provide dialysis patients, staff, and the public a safe, functional, and comfortable treatment environment. This Standard is not met as evidenced by: 50873 Based on observation, interview, facility document review, and facility policy review, the facility failed to label dialysis machines awaiting repair to prevent use for 1 of 1 NxStage System 1 hemodialysis machine and 1 of 1 NxStage Versi hemodialysis machine located in the biomed room. This had the potential to affect 92 of 92 patients receiving incenter hemodialysis. Findings Include: Review of a facility policy titled, “Use of Hemodialysis & Ancillary Machine Repair/Return Tag,” dated 11/04/2019, revealed, “The Standard Repair Request tag will be filled out and affixed to the hemodialysis machine or ancillary equipment whenever it is removed from service.” Further review revealed, “The Standard Ready for Use tag will be filled out and affixed to the hemodialysis machines or ancillary equipment whenever a machine is ready to return to service.” During an observation and concurrent interview on 05/21/24 at 10:35 AM, a NxStage System 1 hemodialysis machine and NxStage Versi hemodialysis machine were observed unlabeled in the biomed room. The Director of Operations stated that if machines were in the biomed area they were not ready for use unless they had ready for redeployment paperwork. During an interview on 05/22/24 at 12:38 PM, Biomedical Technician D stated that NxStage machines in the biomed room were not ready for use unless they had redeployment paperwork. During an interview on 05/22/24 at 3:30 PM, Clinical Manager (CM) B confirmed that there were patients in the Transitional Care Unit (TCU) at the facility that received incenter dialysis on those types of machines. She stated that there was no policy regarding the use of equipment repair tags for the TCU specifically but did confirm that there was an incenter policy regarding the use of equipment repair tags. CM B stated that there was no current labeling system to indicate if NxStage machines off the clinic floor needed servicing.”
