What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Environment: Secure And Restricted
The inspectors’ words, 289 words
“ENVIRONMENT-SECURE & RESTRICTED CFR(s): 494.40(a) 8 Environment: secure & restricted The water purification and storage system should be located in a secure area that is readily accessible to authorized users. The location should be chosen with a view to minimizing the length and complexity of the distribution system. Access to the purification system should be restricted to those individuals responsible for monitoring and maintenance of the system. This Standard is not met as evidenced by: 35966 Based on observation and interview, the facility failed to ensure that access to the water purification and storage system was secure at all times. The findings include: On 7/21/2025 at approximately 8:20 AM, it was observed that a door on the treatment floor was propped open. This door provided access to staff offices and to a second door leading into a hallway. The second door, which provides access to the hallway leading to the water purification and storage system, was also observed to be propped open. The third door, which provides direct access to the water purification and storage system room, was observed to be closed but not secured (unlocked) at the time of the observation. During the interview with the Biomedical Technician on 7/21/25 at 8:32 AM, it was confirmed the room was left unlocked by mistake and should always remain secured. The facility policy titled “Water Purification System Security” last revised on 10/27/20 states “The Water Purification System will be kept secure and entry will only be allowed to individuals responsible for monitoring and maintenance of the system. Only authorized personnel will have access to the Water Purification System’s room. The door to the Water Treatment room must be kept locked, except when there are authorized personnel working in the room.””
