What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Dialysate Proportioning - Monitor Ph/Conductivity
The inspectors’ words, 404 words
“DIALYS PROPORT-MONITOR PH/CONDUCTIVITY CFR(s): 494.40(a) 5.6 Dialysate proportioning: monitor pH/conductivity It is necessary for the operator to follow the manufacturer’s instructions regarding dialysate conductivity and to measure approximate pH with an independent method before starting the treatment of the next patient. This Standard is not met as evidenced by: 35966 Based on record review and staff interview, the facility failed to ensure that four of ten sampled in-center hemodialysis patients’ dialysate conductivity was within acceptable parameters before beginning dialysis treatment. (Patients #3, #4, #8, #9) The findings include: 1. A review of the Hemodialysis Flowsheet dated 1/20/24 for Patient #3 showed the meter conductivity reading was 14.0 and the theoretical conductivity (TCD) was 13.5. The Hemodialysis Flowsheet dated 2/20/24 for Patient #3 showed the meter conductivity reading was 14.0 and the TCD was 13.5. 2. A review of the Hemodialysis Flowsheet dated 2/3/24 for Patient #4 showed the meter conductivity reading was 14.0 and the TCD was 13.6. The Hemodialysis Flowsheet dated 2/6/24 for Patient #4 showed the meter conductivity reading was 14.0 and the TCD was 13.6. 3. A review of the Hemodialysis Flowsheet dated 2/2/24 for Patient #8 showed the meter conductivity reading was 13.2 and the TCD was 13.7. The Hemodialysis Flowsheet dated 2/5/24 for Patient #8 showed the meter conductivity reading was 13.2 and the TCD was 13.6. The Hemodialysis Flowsheet dated 2/12/24 for Patient #8 showed the meter conductivity reading was 13.2 and the TCD was 13.6. The Hemodialysis Flowsheet dated 2/14/24 for Patient #8 showed the meter conductivity reading was 13.0 and the TCD was 13.6. 4. A review of the Hemodialysis Flowsheet dated 2/22/24 for Patient #9 showed the meter conductivity reading was 13.9 and the TCD was 13.5. The Dialysis Clinic, Inc policy titled “pHeonix Meters” effective December 19, 2019 states “Calibration of the conductivity and pH functions of the pHeonix meter will be verified daily, prior to use or whenever inaccurate readings are expected. Dialysate conductivity, temperature, and pH readings will be obtained and compared to the dialysis machine readings PRIOR to the initiation of each hemodialysis treatment. Temperature +/- 1 degree Celsius in comparison to machine. Conductivity +/- 0.3 of TCD (theoretical conductivity).” During an interview with the Clinic Manager on 2/27/24 at 12:10 pm, she confirmed that the difference between the conductivity meter and TCD should not be greater than +/- 0.3. She stated the each patient’s hemodialysis treatment should not have started.”
