What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Environment: Secure And Restricted
The inspectors’ words, 238 words
“ENVIRONMENT-SECURE & RESTRICTED CFR(s): 494.40(a) 8 Environment: secure & restricted The water purification and storage system should be located in a secure area that is readily accessible to authorized users. The location should be chosen with a view to minimizing the length and complexity of the distribution system. Access to the purification system should be restricted to those individuals responsible for monitoring and maintenance of the system. This Standard is not met as evidenced by: 35966 Based on observation, staff interview and policy review, the facility failed to maintain restricted access to the water treatment area. The findings include: At 8:55 am on 5/13/24, it was observed that the door leading to the water treatment area was unlocked. This allowed unauthorized persons to have access to the Water Treatment Room. During an interview with Staff #A on 5/13/24 at 9:00 am, when asked about the door being unlocked it was stated that,”Sometimes it locks and sometimes it doesn’t.” The facility policy titled “Physical Security and Facility Access” published 9/5/23 states,”Restricted access must be maintained to prevent unwanted tampering with the water treatment equipment. The facility must be maintained to prevent unwanted entry by outside persons not involved in the daily operation of the clinic. This is accomplished by: Locking all doors that allow access to the water treatment equipment. No exterior building door will be left unlocked unless under the continual supervision of a facility staff member.” .”

Pe - Safe, Functional, Comfortable Environment
The inspectors’ words, 194 words
“PE-SAFE/FUNCTIONAL/COMFORTABLE ENVIRONMENT CFR(s): 494.60 The dialysis facility must be designed, constructed, equipped, and maintained to provide dialysis patients, staff, and the public a safe, functional, and comfortable treatment environment. This Standard is not met as evidenced by: 35966 Based on observation and staff interview, the facility failed to maintain a safe environment for its patients by not ensuring the door to the dialysis treatment floor was closed and secure. The findings include: Observation on 5/13/24 at 8:50 am of the dialysis clinic waiting area revealed the door to the patient treatment floor was propped open with a door stop. There were no staff present at the front desk to prevent unauthorized individuals from gaining access to the treatment floor. An interview with the Clinic Manager on 5/13/24 at 9:35 am confirmed the door had been propped open due to staff assisting a patient to their dialysis treatment station and had failed to close and secure the door behind them. The facility policy titled “Physical Security and Facility Access” published 9/5/23 states “Doors from the waiting area to the treatment area should remain closed and locked at all times while still allowing emergency access/exit.” .”
