What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Jun 10, 2025OCCL Standards Complaint: No Evidence to SubstantiateOn May 8, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of failing to notify OCCL of an injury requiring medical attention, failing to inform a parent immediately of a serious injury, failing to provide a written injury report within the required time frame, failing to follow the center s policies regarding routine and emergency health care, and not being truthful when providing information to parents, guardians, or OCCL. An investigation was begun on May 14, 2025, and concluded on June 2, 2025. Based on information obtained through interviews with staff and the administrator, as well as a review of center policies, injury reports, and child files, there is no evidence to substantiate this complaint for any of the allegations listed above. While the injury did occur at the child care facility, it occurred after the child had been signed out of the facility by Relative #1. When the center was notified there was an error in the original injury report, they corrected the report and sent a corrected copy to the parent. The center was in compliance with regulations related to reporting injuries, communicating with parents and guardians, maintaining documentation, and adhering to health and safety procedures.
Mar 27, 2024OCCL Standards Complaint: SubstantiatedOn February 29, 2024, the Office of Child Care Licensing received a complaint regarding incorrect staff-to-child ratio, unqualified staff alone with children, and inappropriate care of children. An investigation was conducted on March 6, 2024. Based on the results of this investigation this complaint is substantiated for incorrect staff-to-child ratios because attendance documents from the week of February 26, 2024, showed several infants who were present in the building but not signed into the opening classroom on multiple days during that week. Also, both the center administrator and Staff #3 stated infants are not being signed into the opening classroom. There is no evidence to substantiate unqualified staff are being left alone with children because all four staff who were interviewed stated they have never observed an unqualified staff member alone with children. There is no evidence to substantiate inappropriate care of children because all four staff who were interviewed stated they have never observed children being allowed to cry or forced to sit against the wall in the lobby area. An additional concern is children s attendance is not being properly documented because staff are not signing infants into the opening classroom. Non-compliance Cited: Reg #43 Ensure children s attendance includes exact arrival and departure times. Attendance documents from the week of February 26, 2024, show several infants were not signed into the opening classroom on multiple days throughout the week. Send two weeks worth of attendance documents to OCCL by April 8, 2024. Reg #27 Ensure the staff-to-child ratio for infants is maintained at all times. The center administrator and Staff #3 both stated the opening classroom is not maintaining proper staff-to-child ratio. Send two weeks worth of attendance documents to OCCL by April 8, 2024.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
