What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Intent to Place on Warning of ProbationDelaware has given notice: Intent to Place on Warning of Probation.
Oct 12, 2026OCCL Standards Complaint: SubstantiatedOn September 29, 2026, the Office of Child Care Licensing (OCCL) received two complaints regarding concerns of an insect infestation within the facility. On October 5, 2026, a third complaint was received on this same subject from a third reporter. An investigation was conducted on October 5, 2026. This complaint is substantiated for an insect infestation based on interviews conducted and documentation reviewed. Owner #1, Administrator #1, and two staff members interviewed stated the facility experienced a roach infestation approximately one month prior to the investigation. Documentation reviewed confirmed the facility contracted with an insect extermination company for extermination services and maintains ongoing pest control treatments. Although LS #1 did not observe any live roaches during the facility walkthrough, the evidence supports that the facility experienced a roach infestation and implemented corrective measures to address the concern.
Nov 25, 2024OCCL Standards Complaint: No Evidence to SubstantiateOn November 1, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of improper staff-to-child ratios in the infant classroom, use of improper discipline, altering attendance records, safety and sanitation procedures in the infant classroom, not notifying families of contagious illness, and not following exclusion policies. An investigation was conducted on November 6, 2024. There is no evidence to substantiate this complaint for staff-to-child ratios in the infant classroom based on observations of the classroom, staff interviews, and a review of attendance records. All staff interviewed stated classrooms are in staff-to-child ratio and attendance records reflected all classrooms in correct staff-to-child ratio including the infant classroom. There is no evidence to substantiate this complaint for use of improper discipline by staff based on interviews conducted. Seven of eight staff members interviewed stated they had not witnessed any improper discipline or participated in improper discipline themselves. One staff member who noted otherwise did not provide more information. There is no evidence to substantiate this complaint for altering attendance records because attendance records were reviewed at the time of this visit and interviews conducted. No attendance records reviewed look to be altered, and all staff interviewed stated attendance records are not altered by anyone including the administration team. There is no evidence to substantiate this complaint for lack of safety and sanitation procedures based on interviews conducted. All staff interviewed stated they follow all safety and sanitation procedures in all classrooms. There is no evidence to substantiate this complaint for not notifying families of contagious illness and not following exclusion policies based on interviews conducted and documents reviewed. All staff interviewed stated families are notified of all contagious illnesses and the letter of notification to families of an illness in the infant classroom was provided and reviewed. There is no evidence to substantiate that a staff member took a child in her personal vehicle to seek medical attention; the ill child was taken home because the family had no means of transportation.
Aug 26, 2024OCCL Standards Complaint: SubstantiatedOn August 8, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of unqualified staff working alone with children, improper staff-to-child ratios, use of improper discipline, no attendance being recorded, and no accident/injury reports being completed. An investigation was conducted beginning August 14, 2024, and concluding August 20, 2024. This complaint is substantiated for unqualified staff members working alone with children because a staff member was changing diapers in a separate area without being determined eligible and without a DEEDS Early Learning Certificate. This complaint is substantiated for improper staff-to-child ratios and exceeding maximum group size based on observation at the August 8, 2024, site visit. The toddler classroom was observed with 13 children present with the youngest child being under the age of one. There is no evidence to substantiate this complaint for use of improper discipline by staff based on interviews conducted. Five out of six staff members interviewed, including administration, stated they had not witnessed any improper discipline. There is no evidence to substantiate this complaint for no attendance being recorded because attendance records were viewed at the time of this visit. However, the attendance records for two classrooms were not accurate due to not having exact times of arrival of the children and children not being signed into the facility. There is no evidence to substantiate this complaint for no accident/injury reports being completed based on observations of accident /injury reports for multiple children. An additional concern noted at the visit was safe sleep violations because two infants were observed sleeping on their stomachs and not being able to roll on their own, and bottom covers were not tight-fitting to the mattress. An additional concern noted at the visit was the kitchen area being unsanitary with no counter space available to prepare food and spoiled food s itting on the counter. An additional concern noted at the visit was indoor safety hazards, including a safety gate not being secured to the wall correctly with nails sticking out the wall, a broken cubby in front of the preschool classroom, and missing and water damaged ceiling tiles. An additional concern noted was the posted lesson plans in two classrooms were not current. An additional concern observed at the follow-up visit was plug-in air fresheners in multiple locations of the facility.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
