What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

DateWhatDetail
Oct 1, 2025OCCL Standards Complaint: SubstantiatedOn July 21, and July 29, 2025, the Office of Child Care Licensing (OCCL) received self-reports from the center regarding two separate incidents of a lack of supervision. On July 29, 2025, OCCL received a complaint about a lack of supervision regarding the July 21, 2025, incident. An investigation began on August 11, 2025, and concluded on September 2, 2025. Based on staff statements, administrative interviews, and observations at the visit, this complaint is substantiated for a lack of supervision. Additional concerns noted during the investigation included a person who had not been fingerprinted for the center present with the children during the first incident, and the center having lunch in a space at a different location that was not approved by OCCL.
Jul 30, 2025OCCL Standards Complaint: No Evidence to SubstantiateOn July 18, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding an allegation of improper discipline. An investigation was conducted on July 23, 2025. Based on staff interviews, there is no evidence to substantiate this complaint for improper discipline. All staff members interviewed stated they have not witnessed or heard of any staff members using improper discipline with any children. Additionally, there was documentation of staff using positive reinforcement with the children.
Nov 21, 2023OCCL Standards Complaint: SubstantiatedOn August 14, 2023, a complaint was called into the Child Abuse and Neglect Report Line and was not accepted for investigation by the Institutional Abuse Unit (IAU). It was sent to the Office of Child Care Licensing (OCCL) on August 14, 2023, and accepted for investigation for improper discipline, not following the center’s policy on social media postings, failure to report suspected abuse or neglect to the Report Line, staff file requirements, not providing an incident report to a parent, and staff not modeling appropriate behavior. This specialist was in communication with the Wilmington Police Department (WPD) regarding the status of their investigation into this incident. This specialist was given permission by WPD to begin an investigation on October 18, 2023. OCCL conducted an investigation beginning on October 18, 2023, and concluding on November 16, 2023. This complaint is substantiated for improper discipline because a video recording showed Staff #1 and Staff #2 using improper behavior management techniques with Child #1. This complaint is also substantiated for staff not following the center s policy on social media because Staff #1 and Staff #2 posted a video to social media of Child #1. The allegation that the center did not notify the Child Abuse and Neglect Report Line is unsubstantiated because LS Flowers verified Ms. Law did contact the Child Abuse and Neglect Report Line on August 10, 2023. There is no evidence to substantiate this complaint for staff file requirements because a review of staff files showed all staff members had completed an annual review of abuse and neglect. There is no evidence to substantiate this complaint for staff not modeling appropriate behavior by using inappropriate language or teasing children because the center administrator and Staff #3 both denied observing any staff member using inappropriate language in front of children or teasing the children. There is no evidence to substantiate the center did not complete an accident injury report because Ms. Law did verbally notify Parent #1 of the incident the same day and had a report available the next morning for signature, but the parent was not present at the center that day.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
