The short answer
Record last released by Delaware Office of Child Care Licensing on Oct 23, 2026.
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

|---|---|---|
| Jun 3, 2026 | OCCL Standards Complaint: No Evidence to Substantiate | On May 7, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding failing to report an injury that required medical attention to OCCL. An investigation was conducted on May 13, 2026. After speaking with the administrator and a staff member, watching camera footage, and viewing clips of the incident showing Child #1 using their arm at the center on the days following the incident, the incident appears to have been accidental in nature and occurred while Staff #1 was performing routine duties. Staff #1 responded appropriately by immediately tending to the child, applying first aid, documenting the incident, and notifying the parent. Ongoing monitoring and communication with the parent were also reviewed. Additionally, video evidence and documentation confirm that Child #1 continued to use their arm in the days following the incident, and there were no immediate indicators of a serious injury at the time. There is no evidence to substantiate this complaint for failing to report an injury that requires medical attention to OCCL. |
| May 20, 2026 | OCCL Standards Complaint: No Evidence to Substantiate with Concern | On April 29, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding incorrect staff-to-child ratio and disrespectful communication by staff. An investigation was conducted on May 13, 2026. Based on staff interviews and documentation reviewed, there is no evidence to substantiate this complaint for incorrect staff-to-child ratio and disrespectful communication by staff. The administrator and all staff members interviewed denied any disrespectful communication by staff. All staff members interviewed stated they are never over proper staff-to-child ratio and attendance records reviewed did not show classrooms over staff-to-child ratio when attendance records were completed accurately. However, an additional concern noted was staff and child attendance documentation was frequently inaccurate. There were multiple instances over five days in multiple classrooms where staff members did not sign in or out at all. There were also three instances over two days where children were either not signed in or not signed out of the classrooms. |
| May 20, 2026 | OCCL Standards Complaint: Substantiated | On April 30, 2026, the Office of Child Care Licensing (OCCL) received two complaints regarding failing to have sufficient staff to fulfill, at a minimum, administrative, fiscal, clerical, cleaning, maintenance, food services, direct child care, and supervisory functions, failing to have a staff member who is qualified as at least an early childhood teacher who is present and designated as responsible for the center when the administrator is not present, and disrespectful communication by staff. An investigation was conducted on May 13, 2026. Based on documentation and staff interviews, this complaint is substantiated for failing to have a staff member who is qualified as at least an early childhood teacher who is present and designated as responsible for the center when the administrator is not present. While staff members stated Staff #1, Staff #2, and Staff #3 would be in charge if the administrator were not present, none of the staff members had documented training to be designated as such. Based on staff interviews, there is no evidence to substantiate the center for not having sufficient staff or disrespectful communication. Most staff stated parents only occasionally need to wait at the door when staff get busy and all staff members interviewed stated they have no concerns about staff members yelling or using disrespectful communication. |
| May 19, 2026 | OCCL Standards Complaint: No Evidence to Substantiate | On April 15, 2026, and April 20, 2026, the Office of Child Care Licensing (OCCL) received two complaints from the same reporter regarding allegations of failing to design effective positive behavioral supports or to make reasonable accommodations to comply with provisions in an IFSP or IEP and to adapt behavior supports or other practices for a child who has a special need or disability. An investigation was conducted on April 22, 2026. There is no evidence to substantiate this complaint for failing to design effective positive behavioral supports or to make reasonable accommodations to comply with provisions in an IFSP or IEP and to adapt behavior supports or other practices for a child who has a special need or disability. The center conducted a meeting with Parent #1 on March 21, 2026, and created an Individual Action Plan (IAP) for Child #1 that was signed by Parent #1. Administrator #1 sat in on an IEP meeting for Child #1 to learn what goals the district has set for Child #1. In addition, documentation in the signed IAP indicates an agreement that Parent #1 would be required to pick up Child #1 in the event redirection does not work. This is consistent with the center s policy related to safety |
| May 13, 2026 | OCCL Standards Complaint: No Evidence to Substantiate | On April 6, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of failure to follow policies for parents access to the center, suspension policy, policy for reporting accidents or critical incidents involving the child and other important information related to the child, policy for handling complaints from parents, failing to consult with a child’s parent or guardian and professionals if necessary to design effective positive behavioral supports or to make reasonable accommodations to comply with provisions of an IEP, staff being disrespectful, and failing to provide at enrollment an opportunity for the parent or guardian and child to observe the center. An investigation was conducted on April 8, 2026. There is no evidence to substantiate this complaint for failing to report critical incidents to the parent, failing to handle complaints from parents, or failing to consult with parents and other professionals if necessary to design positive behavior supports. The center has a policy regarding behavior management, including documentation of behaviors and parent notification. The center provided seven documented behavior reports over a six-day period for Child #1 that were not signed by the parent, as well as email communication to Parent #1 regarding concerns, resources, and requests for meetings. Documentation reviewed shows the center informed Parent #1 of concerning behaviors and attempted to address concerns through a scheduled meeting and the development of an Individual Action Plan (IAP), which was signed by Parent #1. There is no evidence to substantiate this complaint for failing to follow their suspension policy as the handbook states, If a child’s behavior poses a risk to their safety or the safety of others, families may be contacted for immediate pickup. In addition, documentation in the signed IAP indicates an agreement that Parent #1 would be required to pick up Child #1 or remain with Child #1 in the event of unsafe behaviors. This is consistent with the center s policy related to safety. Regarding denial of access to video footage, the handbook states parents may not view footage in order to protect the privacy of other children. There is no evidence to substantiate this complaint for allegations of staff being rude or disrespectful. Administrator #1 stated neither they nor Administrator #2 were rude or disrespectful to Parent #1. There is no evidence to substantiate for failing to provide, at enrollment, an opportunity for the parent or guardian and child to observe the center. Administrator #1 stated Parent #1 was given a tour prior to enrollment. |
| Apr 29, 2026 | OCCL Standards Complaint: Substantiated | On April 7, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of failing to follow suspension policy, failing to allow parents to access the facility, behavior support requirements, and caring for children in inappropriate indoor space. An investigation began on April 8, 2026, and concluded on April 15, 2026. After interviews with the administrators, a review of the parent handbook, and a review of information from the center’s communication application, this complaint is substantiated for caring for children in inappropriate indoor space. Administrator #2 stated they were caring for children in the office on April 7, 2026, due to another staff member calling out. There is no evidence to substantiate this complaint for failing to follow their suspension policy. Child #1 was withdrawn from care by the parent; the center did not suspend the child from care. There is no evidence to substantiate this complaint for failing to allow parents to access the facility; the parent handbook states parents may enter the classrooms during drop off. There is no evidence to substantiate this complaint for failing to provide appropriate behavior support. As Child #1 was disenrolled by their parent on the second day of care, there was no time for a meeting to be held to discuss the concerns and create an action plan. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
