What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

DateWhatDetail
Jan 22, 2026OCCL Standards Complaint: SubstantiatedOn January 5, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding not maintaining required temperatures, not notifying OCCL of an equipment failure, and air quality. An investigation was conducted on January 6, 2026. The complaint is substantiated for not maintaining required temperatures, not notifying OCCL of an equipment failure, and unacceptable air quality because staff admitted the building s propane was out and the building was not maintaining required temperatures, classroom temperatures ranged between 61 degrees F and 62 degrees F, OCCL was not notified of the equipment failure, and a strong gas-like odor was present. A concern noted was OCCL was not notified of the hiring of Administrator #2, after the resignation of Administrator #1.
Nov 25, 2025OCCL Standards Complaint: SubstantiatedOn November 1, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding enrollment, child attendance, not providing truthful information, using an unapproved outdoor area, using wading pools, not following meal pattern requirements, ratios, group sizes, mixing age groups, and not having extra clothes in case of an accident. An investigation was conducted on November 12, 2025. The complaint is substantiated for using wading pools because Administrator #1 admitted the school-age children used portable wading pools for a variety of water play activities. There is no evidence to substantiate the complaint for enrollment, child attendance, not providing truthful information, using an unapproved outdoor area, not following meal pattern requirements, ratios, group sizes, mixing of age groups, and not having extra clothes in case of an accident because staff denied the allegations. Additionally, child file documentation was provided for Child #1; reviewed attendance records showed appropriate staff-to-child ratios, group sizes, and mixing of age groups, menus with appropriate serving sizes were observed; and a stock of extra clothes and shoes was observed.
Feb 18, 2025OCCL Standards Complaint: No Evidence to SubstantiateOn February 4, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding unsafe air quality. An investigation was conducted on February 5, 2025. There is no evidence to substantiate for unsafe air quality because Administrator #1 stated she contacted the fire department, and the fire department evaluated the building and found no gas leak. Administrator #1 informed families of the status via Brightwheel.
Jul 30, 2024OCCL Standards Complaint: SubstantiatedOn June 26, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding children being improperly restrained in vehicles while being transported by center staff to field trips for their summer camp. An investigation was conducted on July 2, 2024, and July 16, 2024. This complaint is substantiated because Administrator #1 stated children ages four through seven use a booster seat, without checking their height or weight, and eight-year-old children were not using any type of restraint aside from the vehicle s seatbelts.
Feb 28, 2024OCCL Standards Complaint: SubstantiatedThe Office of Child Care Licensing (OCCL) received a complaint on December 7, 2023, regarding improper staff-to-child ratio, not having someone trained in the day-to-day operations, and transportation requirements. An investigation was conducted on February 15, 2024. This complaint is substantiated for improper staff-to-child ratio because, according to attendance records, on November 29, 2023, Staff #2, an aide who was not yet determined eligible, and Staff #3, an aide under 18 years old, were alone in the two-year-old classroom with up to nine children from 8:04 am until 8:30 am. There is no evidence to substantiate this complaint for transportation procedures or not having a staff member trained in the day-to-day operations because the administrator stated children are transported in the center van, not personal vehicles, and the center has multiple staff trained in the day-to-day operations. A concern noted was Staff #2 was present in the center before her eligibility results were received.
Feb 19, 2024OCCL Standards Complaint: SubstantiatedA self-reported complaint was received by the Child Abuse and Neglect Report Line on December 7, 2023, and accepted for investigation by the Institutional Abuse Unit (IAU). The complaint was sent to the Office of Child Care Licensing (OCCL) and accepted for investigation regarding improper discipline. On February 8, 2024, IAU stated OCCL could begin their investigation. The IAU case was substantiated for physical abuse. OCCL conducted an investigation on February 15, 2024. This complaint is substantiated because Staff #1 was observed on video improperly disciplining several children. When the administrator was made aware of the allegations, she reported the incidents to the Child Abuse and Neglect Report Line, law enforcement, and OCCL. Staff #1 s employment was immediately terminated. Non-compliance Cited: Reg #20: Ensure positive behavior supports are followed at all times. Staff #1 was observed on video improperly disciplining and interacting inappropriately with several infants. This was corrected when Staff #1 s employment was immediately terminated and the administrator reported the incidents to the Child Abuse and Neglect Report Line, law enforcement, and OCCL.
Feb 14, 2024IA Investigation: SubstantiatedDelaware gives no summary.
Dec 13, 2023OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on December 1, 2023, regarding a child being present who is not enrolled. An investigation was conducted on December 5, 2023. There is no evidence to substantiate this complaint because all infants had complete child files and were enrolled.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
