The submitter is reporting the unit for posing an injury hazard.

At the ER they stitched her wound and took x-rays.

Her son sent a letter to the firm reporting the incident. She said the firm has not yet responded.

The product, in the report: WARRIOR 4 1/2” CUT-OFF WHEEL, 40 GRIT,ALUMINUM OXIDE, METAL CUTTING, ITEM 61195

On May 31, 2016, at my home, while using a Makita electric angle grinder with a 4.5 inch metal cutting disc to cut metal, the Warrior brand (sold by Harbor Freight Tools) cutting disc shattered. I had installed a new cutting disc prior to beginning this cut. The metal I was cutting was secured in a Black &Decker Workmate bench as seen in the photos. After having cut approximately 1 inch in to the metal the cutting disc disintegrated causing the tool to rebound against my right leg causing a 4 inch laceration to my right thigh. Depth of the laceration was more than 1 inch. I was home alone at the time so it was necessary for me to call 911 to be transported by ambulance to my local hospital emergency room in Winamac, IN. I was seen by the ER doctor initially who referred me to a surgeon for treatment of the wound. I was taken to surgery where the wound was cleaned, debrided and closed with several layers of stitches. After the post-op recovery I was allowed to return home. I have used dozens of these type discs over the years and have never had a problem. I suspect that there was some kind of manufacturing defect that caused this one to shatter.
