I walked past a USB adapter plugged into the wall. When I bumped it, most of it fell off, but the PLUG stayed in the socket, leaving two exposed terminals sticking out (an immediate electrocution hazard). On inspection, this design that can swap out prongs for different types of socket is extremely hazardous and liable to fail in exactly the same way for other consumers
