The product, in the report: AC Adapter
The consumer indicates that she charged her Galaxy Book Pro today for the first time for approximately thirty minutes. When the consumer went to disconnect the ac adapter from the wall, she was shocked to find out just how extremely hot it was to the touch. It took roughly fifteen minutes for the unit to cool down enough for the consumer to pick it up again.
The consumer notes that the heat was only limited to the ac adapter and that her Galaxy Book Pro works perfectly fine.
The consumer plans on keeping her ac adapter for at least the next thirty days in case an investigation ensues however she has ceased use of it.

The product, in the report: The product is a charger/adapter for a Samsung Galaxy Note 9 Model#SM-N960U
My wife,[REDACTED] and I purchased two (2) Samsung Galaxy Note-9 phones in November, 2018 from Best Buy in Lakeland, FL. We entered into a two-year contract with Sprint. We each received a charger. During the week of July 20, 2020, my wife began getting shocked while she was talking on phone, with the charger plugged in.
After examining the charger, we discovered that the wire was frayed, with the copper wiring completely exposed. My wife reached out to Samsung Corporation who only informed her that the phone and charger were no longer under warranty. However, when my wife put me on the phone, I informed the person, at an off-shore call center, that this was an existing hazard Samsung needed to address. I requested that someone in authority call me back or else I would file a grievance through the Consumer Product Safety Commission. I did receive a return call the following day, from a woman who said I should speak with someone from their “Product Liability” office. I informed this person that I did not have the time since the return call was received before the close of business. I wanted to reach out to you first before contacting Samsung. As previously stated, my wife sustained several painful shocks. Please note she has a formerly diagnosed cardiac condition that necessitated her being placed on disability nearly a decade ago.

The product, in the report: Samsung USB Travel Adapter Model: EP-TA50JWE 5.0V 1.55A
I have two Samsung USB 1.55 Amp chargers that are used to charge two separate Samsung cell phones. Both the chargers came with the Samsung phones. I have had them for approximately 1 to 2 years. Yesterday (12/20/19) I discovered that both chargers have yellowing discoloration and part of the exterior plastic coating of the charger has melted off. I am not sure exactly when the yellowing and melting started, probably in the last 30 days. They have both been only used in our house which was built in 2013. I Feel this could be a fire danger if they are left unchecked in power outlets. The description on the adapters says, Samsung Travel Adapter, Model: EP-TA50JWE. Serial number for the first travel adapter: DK5k615VS/A-E Serial number for the second adapter: DW2J901CS/A-E. I contacted Samsung today (12/21/19) at 1-800-726-7864, who said they would document it.

The product, in the report: android metal charge 9ft cord
I bought a new samsung Galaxy 6 and along with accessories. The new 9ft metal mutli-color charging cord was plugged into phone and charge strip i had laying on my bed. I was cleaning and put an electric item on bed and the metal of plug hit the metal of cord and immediately caused a fire and as I grabbed it within seconds I was shocked and able to put out the fire an actual fire. It burned approximately 2-3 inches of the metal off cord and you can see the burn on plastic. I have no idea if item still works. If a child uses this for an electronic device and doest pay attention with other cords n area.....a house can burn down. I can send photo if requested.

The product, in the report: Samsung Fast Wireless Charging Stand (Model# EP-NG930) (new in Box from Best Buy)
I received as a gift from my sister on Christmas Day (so I have had it six days) and used on our kitchen counter to charge our Galaxy S7 phones. It was purchased from Besy Buy. On Jan 1 I smelled something burning and walk over to see the charger smoldering at the usb port. There was no phone on it charging. I learned that the blue light means charging. When I walked up to it, that light was on. There had not been a phone on it for at least thirty minutes. I am guessing it burned itself up continuing to charge with nothing there. Good thing I caught it before it caught my house on fire... We use all Samsung OE products because I know that’s important and have two “slow” wireless chargers that have worked perfectly for over a year. All samsung products I own (phone, chagers, etc...) are all within Samsungs 1y warranty.
No injuries, I caught it before it caused damage to other persons/items.
Samsung refused to offer assistance without me providing them the product. I noted that I would only provide the product to the CPSC. I have called and notified Samsung but based on the recent history of items I have declined to send the device to them but instead prefer to send it to your office for testing. Please contact me and I can send you photos for preliminary review. The outlet the device was plugged into is a 20A/125V GFCI in our kitchen, fully compliant with local electrical code, as is the remainder of our home’s electrical system. Our home is located in Cobb County, Ga.

The product, in the report: Samsung Travel Charger (all info included in first page of report)
Product Information: Samsung Travel Charger Model: ETA-U90JWE Input:100-240V-50-60Hz 0.35A Output:5.0V-2.0A
After about thirty (30) minutes of first using the charger, it started glowing (orange-like) internally and making a buzzing or hissing sound. Then it sounded like it popped and some smoke came out, along with a strong odor of burnt wire. These (2) were Christmas presents for my grandchildren and were purchased online at Walmart. The other charger was immediately retrieved and is not being used.
