The product, in the report: The product is the provided NiMH battery charger for the Motorola TALKABOUT T600 H2O FRS radios. These are lime green colored portable radios that fit in most people’s hands.
I was in the process of recording a short training video on how to operate a Motorola T600 FRS radio. I had one end of the battery charger plugged into a wall outlet in my office and the other end plugged into the radios micro-USB port to charge the battery. When I attempted to remove the plug from the wall outlet, the plastic shell pulled apart without much effort and left half of the unit still plugged into the outlet with the two metal spades readily exposed. This created a significant shock hazard to anyone who may now touch both of the conductors stuck in the electrical outlet while trying to extract it. The power circuit would need to be powered down before attempting. The two halves of the plastic cover enclosing the small circuit board seems like it was barely held together and should be more robust.

The product, in the report: UL-US-Listed Motorola cellphone charger (included with Moto G7 Play). Black. approx 1” x 2” x 2” with two AC Power prongs on one end and a USB female connector on the other end. The product appears to be Motorola Model SC-41.
Regarding: Fire and smoke from UL-US-Listed Motorola cellphone charger (included with Moto G7 Play). Last night I woke up to a sound next to my bed. As I was looking, a 2-inch flash of fire occurred, and enough smoke to make the house smell like smoke. The charred UL-US-Listed Motorola cellphone charger ended up against the wall, with the 2 prongs still sticking out of the wall socket. Note: The original CE-listed Motorola cellphone USB cable was attached to the charger, but no device was attached to the USB cable.
