What happened that made you want to report? I purchased a 2-ton gantry crane and separately sold safety accessories (wheel locks and swivel locks) recommended by the supplier. After assembling the crane and installing the components as instructed, I found that the wheel locks failed to immobilize the crane safely, even without a load. Despite weeks of back-and-forth communication, the supplier’s only solution was to send bent replacements, which still did not function properly or meet safety standards. Given the critical safety risks involved, I feel it is necessary to report this issue to prevent potential harm to other users. What type of product(s) were being used? A 2-ton gantry crane, along with wheel locks and swivel locks that were recommended and sold separately for safety. Who was using the product(s)? The crane was being used in an industrial setting by trained personnel. How were the product(s) being used? The crane was assembled and equipped with the necessary accessories for safe operation in a shop environment. The wheel locks were installed according to the manufacturers specifications but failed to properly secure the crane. Were there any injuries that happened or may have happened while using the product? No injuries occurred, but the failure of the wheel locks creates a significant safety hazard. If the crane were to move unexpectedly due to faulty locks, it could result in serious injury or property damage. How did any injuries occur? While no injuries occurred in this instance, improperly secured heavy equipment poses a serious risk of crushing injuries, falls, or uncontrolled movement that could endanger personnel.
