The product, in the report: While sanding, the plastic base of the sander completely broke apart and all the metal pieces flew out of the sander and completely broke apart.
Makita Sander Model BO4556 plastic broke apart and failed prematurely almost causing an injury. Ive searched for recall issues but only on later models and this sander is not that old and not often used compared to other manufacturers or models Ive used in the past.

Hurt, no treatment Made Nov 19, 2012, sent to the company Dec 10, 2012, published by CPSC Dec 26, 2012
The product, in the report: Makita Orbital Sander Model BO5000
I was using the Makita Orbial Sander Model BO5000 when without warning the sanding pad (the pad the sand paper adheres to) spontaneouly and violently frqgmented into 8 pieces. One piece hit my lower right side abdomen below the belt line. That was close! It felt like I had been hit by a bullet. I was wearing heavy pants at the time so my only injury was a bruise about 1” in diameter. It was an alarming experience. I found on-line that this Model was subject to a recall in 2003. I intend to call Makita to see about getting a repalcement pad of better construction.

Hurt, no treatment Made Jun 21, 2012, sent to the company Jun 28, 2012, published by CPSC Jul 13, 2012
The product, in the report: Makita BO5000, 5” random orbital sander, Serial No. 128307 / 93.11
On 06/19/20012, while sanding with my Makita BO5000 random orbital sander (Ser. No. 128307 / 93.11), the pad came apart and sent 8 to 10 pieces of shrapnel flying at high speed. These pieces ranged in size from pencil-eraser size to about 1 1/2” x 1/2”. I was wearing shorts and was struck on the leg by at least one chunk that did not penetrate the skin but really stung and left a small bruise. The failure affected about half the circumference of the pad at the outer edge.

First aid Made Jan 14, 2012, sent to the company Jan 24, 2012, published by CPSC Feb 7, 2012
The product, in the report: Makita Bo5001 orbital sander
Makita orbital sander Bo5001. The rubber pad broke apart while in operation throwing chunks of rubber all around. One hit me in the face -- fortunately I was wearing safety glasses and only suffered minor injury. I wish I had known that there was a recall on this product for this very problem.

Emergency room Made Jun 22, 2011, published by CPSC Jul 14, 2011
The product, in the report: Belt Sander
I have regularly used tools such as table saws, band saws, and chain saws for more than 40 years without incident so I was astonished when I partially amputated the tip of my left ring finger with a belt sander. While sanding, my finger barely touched the part of the belt where it runs back up into the sander. This caught my finger and drew it up into the sander and almost completely amputated the tip. All power tools are dangerous, but I always thought that the worst injury from a belt sander would be an abrasion type injury, certainly not an amputation. I think that an amputation warning label or embossed lettering on this area of the sander would prevent this from happening to someone else.
