The product, in the report: The product is a Performax Combination Alarm with Voice Smoke and Carbon Monoxide detector.
On February 13th, my elderly mother experienced a carbon monoxide incident in her home. She has multiple combination smoke/carbon monoxide detectors installed throughout the residence. The source of the carbon monoxide was an unopened fireplace flue. Two of the detectors involved in this event were Performax model GS886 units. Neither of these units activated during the incident. A third detector, manufactured by [REDACTED], did alarm, allowing her to evacuate safely. This failure is especially concerning because the two Performax detectors were located closer to the fireplace than the [REDACTED] detector that successfully detected the carbon monoxide. On Saturday 2/27/26 we took the Performax detector to the [REDACTED] Fire Department and asked if they had a way to test the carbon monoxide detector function. They tested the detector and it failed to alert. They told us to return it to Menards.
