The product, in the report: ADT home monitoring system, including ADT-supplied Honeywell brand Carbon Monoxide alarm, model SiXCOa
ADT-supplied Honeywell Carbon Monoxide detector [model SiXCoa] failed to go off during a relatively high carbon monoxide leak, ADT monitoring/transmission equipment failed to detect any issues, and no alarms went off. Secondary [REDACTED] brand detectors alerted us to the issue, we evacuated the house and called the fire dept before any injuries were sustained. If we did not have backup [REDACTED] alarms, injuries could have been substantial/death. ADT alarms were directly over the leaking appliance, and backup [REDACTED] alarms were ~20+ft away. ADTs systems failed from point of origin all the way to multiple redundancy monitoring systems. ADT system was installed brand new 2 months prior.
