The product, in the report: Kidde Fire Away Model FA240HD 2A 40 BC Dry Agent Fire Extinguisher Serial Number JA-987288 Date code 340030
On 22 Dec 2018 at approximately 1830 PST a Kidde Model FA240HD 2 A 40 BC fire extinguisher discharge valve failed catastrophically discharging about 6 lbs of Monoammonium Phosphate extinguishing agent into an occupied approximately 15’X15’ indoor area. The valve sheared off below the neck of the cylinder when the cylinder fell onto it’s side when it was bumped by the occupant. The cylinder rocketed a short distance, struck part of the structure, and bounced into a crevice between the structure and a step..
The occupant was exposed to a stream of the agent that struck them in the face causing eye and respiratory irritation. Non-professional first aid consisting of eye flushing and body washing in a conventional shower was conducted. Non-prescription eye drops, saline nasal rinsing, and a prescription rescue inhaler (albuterol) were utilized. Individual carries a rescue inhaler due to chronic asthma.
