No one hurt Made Jan 6, 2018, sent to the company Jan 17, 2018, published by CPSC Feb 1, 2018
The product, in the report: First Alert , Model FE1A10G15, fire extinguisher . Red tank with black plastic handle , red nozzle, small charge indicator ( yellow needle w/ red and green zones) .
On 01 JAN 2018, I had preheated my [REDACTED] propane fueled grill to cook some lamb steaks . My 22 year old grandson & I heard a loud popping noise, followed by an intense grease fire with yellow flames at the rear of the grill . Grill is built into a concrete block island located away from any flammable structure.
We immediately removed the First Alert , Model FE1A10G15 , NO. AB-170560, from its wall mount . We verified that the gauge was “in the green” and attempted activation . There was no sound of escaping propellant and no extinguishing agent was expelled and indicator needle went into the “red” .
Fire intensified, and we immediately got the Model [REDACTED] extinguisher from the garage . Extinguisher functioned as designed and all flames were extinguished within a few seconds .Gas supply was turned off and grill permitted to cool .
The malfunctioning FE1A19G15 caused a delay in extinguishing the blaze . Further, the gauge indicated that it was serviceable . The mount was on the wall of a covered porch , behind an arch - protected from the elements and direct sunlight . Storage temperatures were not exceeded .
Incident was reported to the manufacturer - no response in 48 hours. Defective item held for investigation.

No one hurt Made Jan 4, 2018, sent to the company May 11, 2018, published by CPSC May 25, 2018
The product, in the report: Houshold Fire extinguisher
The consumer tried the extinguisher in the past and it would hurt his finger to push the activation button down, but it would pop back up afterwards. This indicated that it would function in an emergency. However, the consumer tried it in December 2017 and it would not pop back up. He does not believe that there is any pressure in the extinguisher any more. Therefore, it would not work in an emergency.
He decided to report it in light of [REDACTED].
Other information on the extinguisher was: V70-78-00 and a number# HR260570.
It was purchased approximately 10 years ago, but the consumer is not exactly sure when or where from.
It does not appear to be included in any CPSC recall.

On Oct 07, 2016 I bought two First Alert Fire Exinguishers, item # 540003, from Costco #642 in Marysville, WA. I found that both had loose, less than hand tight, hoses at fitting going into the can. The hose could possibly come off while in use.

The product, in the report: Aerosol Spray Can (Extinguisher)
The consumer says that on January 5th the extinguisher suddenly exploded along the base seal. It rocketed approximately 30 inches and lodged 4 to 5 inches deep into a drywall ceiling. There is a 2 1/2 inch hole in the ceiling. It sprayed the contents throughout the room. There were no injuries. The spray can was not in use at the time of the incident as this occurred simply on its own.
