  1. No one hurt

Made Mar 25, 2015, sent to the company Apr 14, 2015, published by CPSC Apr 28, 2015

The product, in the report: Ivory colored bear head with plush/lovey tiny blanket for body/legs. 18inches by 8inches (estimated) Tiny blanket has knotted ends. Carter’s FTY#0914 STYLE#L31475H 04/15/2014 PO#3061

My five month old daughter was on her back on her playmat with the toy suspended above her when my husband noticed she was making a strange choking noise. He pulled the you away from her and noticed that one of the knotted ends of the toy was deep in my daughter’s throat and she was choking. If he had been less attentive, or the toy not attached to a suspension bar (which prevented it from lodging even deeper) she may have choked to death as air could not pass by the knot that is the size of a child’s esophagus.the toy tag says: Carter’s FTY#0914 STYLE#L31475H 04/15/2014 PO#3061 It is a bear head and arms with a lovey blanket around it and the four corners of the blanket are knotted.
