I was using a kintron kb-1 e186244 extension cord. Had it plugged into a wall socket & as soon as I plugged my iPhone in, the extension cord outlet where I plugged my phone shot out sparks, then a small fire started. My iPhone is now fried & won’t turn on along with my tablet that was plugged into the socket as well. I was shocked in the process of trying to unplug it from the wal & have injuries.

The product, in the report: White flat plug 3 outlet extension cord
My 9 year old daughter was bringing me our vacuum cleaner. She didn’t realize it was still plugged into the extension cord. When the cord reached the end of it’s length, the male portion of the vacuum pulled out of the female receptacle and it broke apart. This exposed 2 metal strips that were connected to the wires. When my daughter attempted to put the receptacle back together, was shocked and burned.

The product, in the report: The data on the extension cord is as follows: Forest green extension cord with three receptacles on female end. Male plug end QF KB-1 E186244 KINTRON F40M-10 Female receptacle end Made in Indonesia E186244 KB-IF UL Tag ISSUE NO BX-7116 13A-125V-1625W E186244 1
While stringing lights on Christmas tree, the 3-socket receptacle end of our holiday extension chord split open due to glue or plastic failure.
This exposed two metal strips that had been inside of the socket block. When my mother attempted to press the halves of the live extension cord together this created a short circuit that tripped the circuit breaker.
The data on the extension cord is as follows:
Male plug end QF KB-1 E186244 KINTRON F40M-10
Female receptacle end Made in Indonesia E186244 KB-IF UL Tag ISSUE NO BX-7116 13A-125V-1625W E186244 1
