Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

The product, in the report: Single Outlet Extension Cord

To who it may concern, My name is [REDACTED]the name of my company is [REDACTED]. I do consulting work for a number of different Asia based manufacturers my telephone number is [REDACTED]. I have been marketing testing products for 25 years and sit on some STP consumer boards at UL. Over the last 3 years there has been a mass exodus in China from listing products with UL over to ETL( Intertek). The reason for this is UL stepped up its after market monitoring of off spec products and went to a three strikes you lose your listings law. Meaning, if you products are found to be off spec in the market 3 times your listings are suspended and you cannot use the UL mark. Please note the three companies listed on my testing reports in China all changed from UL to ETL over the last 3 years. There products are in 10,000 Ace Hardware stores Wal-Mart and on line with Harbor Freight and in the 800 stores. After many months of presenting my testing data and making complaints at ETL they refuse accept my complaints which should be against the law. They are not investing cheating on products at all and should be suspended as a safety testing agency. These products are below spec making their maximum amperage capacity and everything listed as per spec false and should all be recalled. I will send you another few reports also as this is not only being done in extension cords but lighting products too. PLEASE TAKE SOME ACTION AGAINST THIS TERRIBLE INJUSTICE TO THE AMERICAN COMSUMER [REDACTED]

The product, in the report: 30ft Retractable Metal Cord Reel w/3 Outlets - 10amp From their site: This Retractable Cord Reel has a 30’ 16/3 SJT Cord. With 3 separate grounded outlets and a 10 amp rating, powering your projects is quick and easy. Simply pull out what you need and when you’re done, all the cord winds back up inside the heavy-duty metal housing. The outlet bar even has a built in LED status light to let you know you have power. Includes mounting brackets that allow it to be attached to the wall or ceiling.

We purchased the Bayco extension cable a few months back for in our garage. On November 12, 2014 we were out there working on a project (thank god we were out there) and my daughter yelled the cord was sparking. We look up to see it spark and smoke. We grabbed the cord to unplug the items (nothing with a major draw, just a 50 w battery charger for a drill and a light, well under the 1250w 10 amp rating) and the cord had melted and broke into 2 pieces, we even had a flying piece of debris hit my son and burn his shoulder. At this point we had the unit unplugged but it was still smoking so my husband pulled it off the ceiling burning his fingers, but it was burning just inches from the Sheetrock. Once down we went to pull the cord out and just saw a melted bunch of extension cord. Right now I am just glad we happened to be working in there when this occurred or there was a very good chance it could have caused a house fire.

I contacted the manufacturer to report this issue and received the following response: “Thank you for bringing this to our attention. Judging from your email, it sounds like you have a retractable cord reel as opposed to an extension cord. One of the Cautions listed in the instructions is that you need to pull out the cord to full length when using the receptacle. If you use the cord for while it is wrapped inside the reel it can overheat and melt the cord. It sounds like this is what happened with your cord reel. Please let me know if this is not correct. We will be more than happy to take the reel back and inspect it to see what happened. Thank you, [REDACTED] Customer Service

Well, no where on the label did it state I had to have the cord fully extended during use and in fact their web site states you can just pull out what you need (I have attached a screenshot of the product description showing this).
