The product, in the report: Mainstay small space junior twin over full low profile metal bunk bed
I am reporting a serious safety incident involving a Mainstays Small Space Junior Twin over Full Metal Bunk Bed purchased from Walmart on July 29, 2025, and delivered August 4, 2025.
On October 10, 2026, the upper sleeping platform collapsed downward while my young son was sleeping on it. My son weighs less than 60 pounds, well below the advertised weight capacity of the upper bunk.
Thankfully, my son was not injured. However, this incident could have caused serious injuries or worse.
The bed is approximately 14 months old and was being used for its intended purpose. I have photographs documenting the damage and proof of purchase.
I am requesting a full refund, an investigation by your product safety team, and a written response explaining how Walmart intends to address this safety incident.
Please provide a case number and instructions for preserving or inspecting the defective bed.

The product, in the report: Mainstays twin over twin pine bunk bed. Bought through Amazon.com
Hello,
I bought a pine Mainstays twin-over-twin bunk bed for my children. My 5 year old is using it right now - my other daughter is not old enough yet. I put the correct size mattress in (7 in.). Twice, my daughter has been lying on the bed and has rolled over and her knee has gotten stuck between the guard rail and middle rail, requiring an adult to dislodge it and causing pain and bruising.
In addition, her foot has gotten stuck between the mattress and guard rail.

The product, in the report: Dorel Asia Bunk Bed Model 2450 (xgx) Pine
My daughter’s bed (lower bunk) side rail split in half and she fell on the floor and ended up with a scratch. I looked at my other daughter’s bed (the upper bunk) and it has a crack on the side rail.
Luckily I was using them as two separate beds.

The product, in the report: Mainstays Twin Over Twin Wood Bunk Bed - dimensions 78.75”L x 43.75”W x 64.80”H this bunk set is designed so that it can also be used at 2 twin beds
My 32 month old daughter fell between the slats on a bunk bed. The slats are to wide apart and she had lifted the mattress and fell in between the slats becoming entrapped and injuring her chest and neck. If I had not heard her shriek when she fell she could have strangled from the pressure on her neck.

No one hurt Made Sep 29, 2011, sent to the company Nov 8, 2011, published by CPSC Oct 31, 2011
The product, in the report: Twin over twin bunk beds, honey pine finish
I have a set of bunk beds in my sons’ room, my older son is 9 and sleeps on the top bunk, my younger son who is 2 sleeps on the bottom. We purchased the beds either late February or early March (My newest baby was born February12, 2011 and I remember taking her to the store when she was between 2-3 weeks old when we purchased this set). My brother and my husband put together the set that night and we placed coil-springed mattresses on them. The FIRST incident: After the beds were up for nearly a week, my older daugther (then 9) was on the top and she must have put too much weight down when she got to the top, I guess and broke one of the support slats under the mattress. I was upset, but did not blame the company because I thought it was her fault and she probably jumped too hard on the top. I didn’t think to report it because my daughter should not have placed herself on the top bunk so firmly. My husband replaced the broken slat on the top bunk wih another piece of the same type of wood, same depth and length, but it is wider so that it is stronger.
The SECOND incident: Just yesterday (9/28/11) probably about 8pm, I was trying to settle down my 2 year old for bed. He weighs 32 lbs, and I am about 130lbs. I believe the weight limit was somewhere between 180-200lbs...but I’m not posiive. I remember the manual saying something about teenager to young adult, so i would assume at least a person of my size would be supported. So anyhoo, I was lying in the bed (BOTTOM BUNK) with my son both our heads at one end on his pillow. He sat up looking for a stuffed animal. I sat up so that I was on the edge of the bed (feet over the side), and he was looking for his animal leaning over the headboard. He reached over the side to get his animal (which was not on the floor but resting on some other stuff that was over the side, so he didn’t have too far to reach), and I heard the board SNAP that was under the matress where his pillow rested; his pillow and his mattress sunk down a little bit. I pulled him off the bed. He was not hurt, thank goodness, but it scared him. I know now that the boards are weak and maybe my daughter didn’t do anything wrong the first time. If a slat breaks when one child is on top and one on bottom there could be serious injuries, or death! The way these slats break are very sharp and could impale my 2 year old!!!
