  1. Death

Made Feb 12, 2017, sent to the company Feb 22, 2017, published by CPSC Mar 8, 2017

The product, in the report: Ashley B102 Lu Lu storage loft bed white in color. Also purchased at the same time was the mattress, roll out slat decking, bookcase, loft stairs, and loft drawer storage.

A loft bed by the name LuLu made by Ashley furniture was purchased from Slumberland furniture in Waite Park MN on 8/31/13. It was also delivered and assembled by Slumberland employees. The incident occurred February 26,2014 which led to the death of a four year old girl. The girl was playing in her room. She asked to watch a movie so her mother started the movie and left her in her room. The mother thought things were quiet but thought her daughter may have fallen asleep. A short while later the mother went to check on her to find her stuck in between the wall and her loft bed. The girls mother attempted CPR while on the phone with 911. The ambulance arrived and transported her to the Litchfield Hospital in MN. The girl died. Meeker County did an investigation to ensure no foul play and found it to be a terrible accident. After this incident occurred I examined the bed to see if there was any product flaws that could be noted. I did find that there is a shelving unit on one end that supported the bed frame that stuck out a little bit. After speaking with her mother she said that one time she had gone behind the bed and had her feet on top of the shelving unit for support. Her mother had to help her get out and told her daughter to never do that again because she might not be there to help her get unstuck. I don’t know what happened the evening when she got stuck which led to her death but I believe she may have tried doing what she had done before but was to far over that her feet did not catch the shelving unit this time and she ended up being hung by the loft bed. It appears that this is a design flaw as there should not be any space that a child could somehow feel safe to stick there legs between a wall and a loft bed. I would also recommend that in the warnings that come with loft beds that they warn parents to make sure bed is flush against the wall or far enough away from the wall that a child may not become entrapped between the wall and the bed. There should also be a warning that placement should not be by a window. Many parents may not think about these things until something tragic like this happens.
