The product, in the report: Sloan Flushmate Pressure Vessel
While away for a few weeks, our toilet began to constantly run and then eventually overflow. This toilet was in our second floor bathroom and ran into the hallway, bedrooms, and soaked right thru the floor. Eventually the floor caved and we now have extensive damage on the main floor as well. Living room ceiling, walls, floors, furniture, and molding.

The product, in the report: Sloan Flushmate ll pressurized flush system
We have a Sloan Flushmate lll and were unaware that this product had a recall. It appears the tank can explode causing the lid and parts of the tank to become projectiles. I noticed my water bill going up and today 10-30-2017 I watched videos on youtube to troubleshoot the issue. I only became aware of the recall by accident after I troubleshot the issue of the toilet hissing (I did not hear it because I am a disabled veteran). I had to put my face next to the tank to find the leak, which I did. It was coming from the top right side of the tank (this pressurized tank is inside the porcelain tank.) There was water squirting to the rear of the toilet tank and that’s when I looked up the part, this is when I found the recall which was dated Jan. 23rd 2014. And it states “Hazard: The system can burst at or near the vessel weld seam releasing stored pressure. This pressure can lift the tank lid and shatter the tank, posing impact and laceration hazards to consumers and property damage.” I had my face on top of this tank !!! They are sending replacement tank but I could have been seriously injured troubleshooting this problem. I never received a recall on this product.

The product, in the report: Sloan Flushmate Series 503 toilet
At 4 pm 9/28/17, we had a Sloan Flushmate Model M-101526-F3 Series 503 toilet explode. There was no one in the room and the water was turned off before considerable damage could take place however, the explosion did have a significant amount of force. The porcelain and tank were shattered. This is very concerning and could of caused great damage to an unexpected patron.
This event happen in a medical office building owned by Tahoe Forest Hospital.

The product, in the report: low volume toilet flush system
re sloan flushmate III series503 one unit burst in 2011. factory replaced at no charge but we had to provide labor to install. now they recall units as a personnel safety issue. factory tells us unit already replaced has no defects but 8 others we have are defective. further they advise a repair kit consisting of a metal strap to be secured around the existing defective tank and a pressure reducing valve will also be supplied. they have redesigned tanks that are not defective so that’s what they should be compelled to send out. the “repair kit” puts a “[REDACTED]” around a defective product and the valve reduces pressure so consumer does not get the flush capabilities of a non- defective product. the CPSC should not save the maker of the defective product but should protect the CONSUMER. you are the CONSUMER psc right?
