First aid Made Sep 24, 2020, sent to the company Oct 8, 2020, published by CPSC Nov 10, 2020
The product, in the report: [REDACTED] Model M-101526-F3 This was a 2-piece toilet in that the separate Tank sat on top of the Base. The Tank and Lid were destroyed in the explosion cased by the toilet’s inclosed Sloan series 503 Pressure Assist mechanism having catastrophic failure.
- On the morning of Tuesday September the 8th 2020, the [REDACTED] toilet that contained a Pressure Assist Flushmate series 503 Sloan power flush Exploded shattering both the Water Tank and Tank Lid propelling shards throughout the bathroom with pieces impacting and cutting my wife’s foot and leg and a large chunk of the Lid clubbing her in the forehead above her Right eye resulting in a nasty goose-egg swelling with associated impact pain from this blunt force trauma injury. One of the large pieces of debris found on the floor was a smooth rounded corner of the Lid which is likely what hit her on the head as there was no external bleeding. Although the pain persisted for days the swelling gradually reduced (leaving a dent in her forehead) and purple and green colors advanced down her forehead and between her eyes. Sudden loud noises would cause her to jump and her heart to race. - It was only after this incident that I was prompted to research this type of product failure and found that the 501, 502, and 503 series Flushmates’ ALL had reported cases of catastrophic failures that destroyed the toilet, caused damage to the home, and exposed the occupants to injury - some who sustained serious wounds. Had I been informed of the known risks of the past 501 and 502 series I never would have purchased 1 [REDACTED] and 1 [REDACTED] toilet with the 503 PA for use in our home. The [REDACTED] toilet that exploded was the newer of the 2. I immediately stopped using both toilets and had them replaced with conventional gravity flush toilets. - During the same time I originally had these toilets installed in my home I also designed a commercial building for the company I worked for ([REDACTED]) and had 8 toilets with the Sloan PA installed. [REDACTED] is now assessing what corrective action is needed since they have been informed they are putting their employees in harms way of random explosion at any time.
