No one hurt Made Sep 12, 2019, sent to the company Sep 27, 2019, published by CPSC Oct 11, 2019
The product, in the report: Flushmate pressure toilet system.
I have a Flushmate toilet system, Model No. M-101526-F3, Serial No. 031507-F3K1-1772, Series No. 503. It came with the condo that I purchased in November of 2013. It worked well until last night. After I flushed, the system made a really loud sound. I took off the lid of the tank and slightly pressed the circular part on the top of the system that looks like a wheel which seemed to release the pressure. I’m trying to either have the thing fixed or, if it is defective and the subject of a recall, I need to have it removed.
I am an 81 year widow and have no idea how to remedy this situation.
Please contact me at [REDACTED]. I am concerned after reading what might happen if this product is defective and might explode.
Thank you.

Hurt, care not known Made Jun 12, 2019, sent to the company Aug 20, 2019, published by CPSC Sep 4, 2019
The product, in the report: Toilet tank
Caller is reporting that toilet tank exploded which shattered pieces becoming a projectile, which could have caused serious injuries. She stated that she was standing in the bathroom when the incident occurred.
Caller feels that this is directly related to recall#16-223. Hazard: The system can burst at or near the vessel weld seam releasing stored pressure. This pressure can lift the tank lid and shatter the tank, posing impact and laceration hazards to consumers and property damage. The manufacturer was not contacted as yet. No one hurt Made Apr 22, 2019, sent to the company May 2, 2019, published by CPSC May 16, 2019
The product, in the report: This is a pressure-assisted toilet unit. It is manufactured by the Flushmate Company separately from the manufacturer of the toilet, which was Crane (now American Standard). Model of replacement unit is M-101526-F3
In March, 2019, I received a recall notice respecting the Flushmate II 501-B units with which our toilets came equipped. Although we had not had any problems with these units during the 20 years we had owned them, we were advised that they were at risk of bursting at the weld seam, which could “lift the tank lid and shatter the tank, posing impact and laceration hazards to consumers and property damage.” I contacted the company on March 7, 2019 and was told that they would send us replacement units for our six toilets that contained the faulty pressure systems, and that the company would send a technician to our house to install the units. The replacements arrived promptly, and the technician came out on April 5, 2019, and installed the units.
On April 20, 2019, my husband used one of the toilets containing the new replacement Flushmate unites, and when he flushed the toilet, the unit burst, spewing water all over the bathroom, and fracturing the china of the toilet, in this case, not the lid but the base.
My husband was not injured, but it is clear that a person might be from this type of explosion. The toilet is of course completely unusable and will have to be replaced.

No one hurt Made Jan 5, 2016, sent to the company Feb 19, 2016, published by CPSC Mar 4, 2016
The product, in the report: Sloan Flushmate model # M-101526-F, Series # 501-B, Serial # 021901-F01-1402 toilet
I purchased a house several years ago. One of the toilets a Flushmate model M101526F, Series 501-B, black pressure vessel installed that is used to flush the toilet. Recently, it started “flushing” on it’s own. I checked the tank and found that it has a small crack in the tank allowing the water to slowly leak out. When the pressure drops enough, it flushes and then refills.
I did some research and discovered a product advisory issued in 2000 for the 501-B series. I also discovered a report on saferproducts.gov (report # 20120818-EB3BD-2147464614). I called the phone number listed at the bottom of the report and the operator confirmed the product advisory and then advised me to contact the Flushmate customer service phone number. I was unable to contact them by phone but sent an email earlier today (1/5/16). Within an hour, a representative called me back and advised me that my tank wasn’t included in the product advisory and told me I needed to purchase another one. I asked about the issue with the tanks cracking / exploding. She told me that mine was out of warranty and I need to buy a new tank. I decided to report the issue here.

No one hurt Made Jun 23, 2015, sent to the company Jul 1, 2015, published by CPSC Jul 16, 2015
The product, in the report: Flushmate® III Pressure-Assisted Flushing System
The Flushmate III Pressure-Assisted Flushing System in the American Standard toilet in my apartment unit exploded. It shot parts of the porcelain tank into hallway. Part of the tank compressor landed 3-4 feet away.
The toilet was not in use at the time, no one was in the bathroom or hallway, and no injuries were sustained. As we were present in an adjacent room, we were able to shut off the water almost immediately. Other than the shattered porcelain toilet tank and destroyed pressure unit, no property damage occurred. We needed to mop up the water and sweep up the porcelain shards. The apartment maintenance team replaced the toilet tank and pressure unit very soon afterward.
It appears the unit was one of those covered under the earlier product recall (i.e. recall number 14-089 dated January 23, 2014 or 12-201 dated June 21, 2012). Apartment building was built sometime between 2003 and 2004.
I’ve lived in my apartment since March 2005 and the toilet’s pressure unit had not been replaced during my residency. There are roughly 100 units in my building and another 100 units in an adjacent building built a year prior to the building I live in. Some units have 2 bathrooms. Maintenance indicated they replaced a pressure unit in another apartment the week previous as the unit cracked, depressurized, and leaked water into the undamaged porcelain tank (i.e. that incident did not result in the porcelain tank rupturing). Maintenance was not aware of the product recall. I provided them the recall information.

No one hurt Made Feb 21, 2014, sent to the company Mar 4, 2014, published by CPSC Mar 26, 2014
The product, in the report: air assisted flushing toilet mechanism, black plastic; Identified as identical to Flushmate system already recalled.
At 3:30am today (2/21/14) while staying at [REDACTED] in [REDACTED], my husband was awakened when the air pressure assisted toilet blew up in [REDACTED]. My husband identified the Flushmate inner mechanism from the recall notice on the internet. It blew the lid off which shattered and broke the toilet bowl. A large shard blew through the open bathroom door and made a dent in the sheetrock of the opposite wall several feet away. It also blew the water line from the toilet & water was rushing everywhere. He shut off the water valve which prevented more damage than could have occurred. My concern as a medical doctor is that this is a public health hazard that is potentially lethal, disabling or disfiguring. I hope your consumer protection organization can help to spread the word that these flushing mechanisms need to be recalled and removed from service before someone is seriously hurt or killed.

No one hurt Made Jul 27, 2013, sent to the company Aug 5, 2013, published by CPSC Aug 19, 2013
The product, in the report: Sloan Flushmate III Pressure-Assist Flushing System: Series 503
On Friday, July 26, 2013 at approximately 1:30 in the afternoon, and about 1 minute after my husband used our guest bathroom toilet (REDACTED) and left the bathroom, we hear an enormous crashing sound.
Ran down the hall to see water gushing out of the bathroom into our hallway. We opened the door to see that the tank of the toilet had exploded into hundreds of pieces. We turned the water valve off and assessed what could have caused this explosion.
I got on the internet and found that the flushing system installed in our toilet, Sloan Flushmate Pressure-Assist Flushing System had caused the explosion.
I was able to get the model, serial number and series of the Flushmate unit and found that it had been under recall for this very same VERY DANGEROUS issue.
I contacted Flushmate and they will replace the entire toilet + labor, once I get a price bid to them (which I emailed this morning 7/27/13)
We have two other toilets in our home with the same Flushmate units under recall and I was notified that they would send two ‘repair kits’ for the other two units. They informed me though, that they would not be responsible for the cost of a plumber to install the “repair kits” and that cost was our responsibility.
After doing further research on the ‘repair kits’ on the internet, I discovered that these were merely a [bandage] on a very dangerous product. Instead of recalling the entire units and replacing them with non-defective new units, we are left with an unacceptable and still dangerous unit.

No one hurt Made May 29, 2013, sent to the company Jun 18, 2013, published by CPSC Jul 2, 2013
The product, in the report: Toilet tank
On March, 31, 2013 (yes, Easter Sunday), approximately five minutes after it was flushed, my toilet tank exploded, spraying water and razor-sharp porcelin shrapnel throughout my bathroom. Fortunately, nobody was in there at the time. I shudder to think of the injuries my 8-year old grandson would have received had he been nearby. Photographs (4) of the result are enclosed. After cleaning up the 3 to 4 gallons of water and the pieces of porcelin on the floor, I discovered that the toilet was a [REDACTED] model with a FLUSHMATE installed in March, 2004, by a licensed plumber. Further, I discovered on the internet that this system had been the subject of a recall notice due to this very problem- a notice that I never received. According to the plumber that I hired, the cost to replace that toilet, in kind, is approximately $750. Having been subjected to your product once, I declined that toilet and instead had a standard non-FLUSHMATE installed. I’m out one very expensive toilet, along with the use of my bathroom for two days, a ruined Easter dinner, and the very real psychological pain of the injuries that were so narrowly averted. I believe you owe me compensation for this disaster caused by your obviously defective product.

The product, in the report: Flushmate model M101526F, Series 501-B, Black pressure vessel for toilet tank.
At around 5:00 am, the downstairs area was found to be flooded with about a half-inch of water. Upon inspection of the women’s restroom it appears that the toilet tank pressure vessel ruptured. The toilet was not in use at the time, and does not see heavy usage. No personnel were injured, however the tank was destroyed along with the vessel scattering shards up to 15 feet away.
After looking up the recall information, this is a different series than is currently under recall for the same issue. This is the Flushmate Model M101526F, Serial # 050498F011838.
This unit is located in a Fire Station along with 8 other of the same model from varying years 1998-2006. This is the first catastrophic failure noted although a few have been replaced over the years.

Emergency room Made Sep 26, 2011, sent to the company Oct 3, 2011, published by CPSC Oct 18, 2011
The product, in the report: Pressurized tank unit for a toilet. Reduces water use.
AFlushmate Flushometer - Tank System, which is a pressurized tank that resides within the toilet, exploded and severely cut my back. A simple internet search shows this has happened to dozens of people. Had it been my grandmother our one of my nieces and nephews who happened to use the toilet that day, the consequences could have been severe. I required dozens of stitches for an extremely deep wound because of the exploding porcelain.
Upon reviewing the Flushmate.com website, there was a small advisory (not a recall) effecting limited quantities of the unit, but considering my flushmate separated in the same manner as the advisory notice mentioned, I feel the issue is larger than the company is letting on.
Because I am a bigger person, I was able to absorb the brunt of the force. Again, had this happened to someone elderly or a child, the outcome could have been catastrophic. There needs to be a larger awareness camaign as we had no idea of the danger involved in using the Flushmate product. I can send pictures of the dammage and my wound.
