The product, in the report: The product is a FLUIDMASTER Toilet Fill Valve 401LG, it looks identical to the model 400 seen here: [REDACTED]

I was repairing our Fluidmaster 401LG Fill Valve, already installed in the toilet tank. The Fill Valve Cap Assembly was faulty and I decided to use a new Fill Valve as a donor for its Fill Valve Cap Assembly. I was sitting on the closed toilet lid, facing the tank, I held the solid section of the new Fluidmaster 401LG Fill Valve Body in my left hand, while pressing and turning the Fill Valve Cap Assembly with my right hand. The Valve Body is height adjustable by a molded threaded section below the solid section, it requires turning like a nut & bolt to change the length. I put the threaded shank end of the Valve Body, on the toilet lid, for extra support. As I was pressing and turning the top Fill Valve Cap Assembly, the entire adjustable section of the Valve Body collapsed. When it collapsed, the skin of my thigh and my pants were trapped in between the upper & lower section of the Valve Body, and the entire Fluidmaster 401LG Fill Valve was attached to my thigh. I was in shock and severe pain, I tried to pull the sections apart, but couldn’t. I tried to twist the sections as one would normally do to adjust the height, but that was even more painful. I was going to call 911, but decided to try once more to pull the sections apart, as I thought of the time it would take for them to arrive and that I would have to walk with the valve attached to my leg, I was finally able to free myself from the assembly. The skin was deeply damaged in a diameter the size of a nickel. The skin was white and the blood was pressed out of the area, I was concerned the section was turned into hamburger and would become necrotic from lack of blood flow and damaged capillaries. I sought medical attention.
