The reports, as CPSC published them

Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

  1. Hurt, no treatment

Made Dec 7, 2025, sent to the company Dec 10, 2025, published by CPSC Dec 29, 2025

The product, in the report: I need to make you aware of a SERIOUS and very DANGEROUS manufacturing/enginnering defect on a product (Bemis Richfield 73088146786 Elongated Toilet Seat) I just purchased on 12/6/25 from The Home Depot (Order #[REDACTED]/SKU #1001262676). On installation, I had missed step 3 after completing step 6, and the hinge caps would not properly seat into the cap. I needed to reverse my steps so that the toilet seat would open properly. I then watched Bemis’s “how to” video for intallation and at the end of the video it advised something to the effect of “don’t worry, you can always loosen (Hardware Kit B) with a 5/8” wrench. When loosening the first “B”, it experienced a CRITICAL FAILURE and EXPLODED the top part of “B” directly at my FACE with such force as to actually lacerate my nose and cheek. Had it been less than 2 inches higher, I would have suffered SEVERE EYE DAMAGE or blindness as I was working directly over “B” when it critically failed. After gathering my wits, I attempted to work on the second “B” but this time covering the top of it in case of a potential repeat. Sure enough, the second “B” also experienced a CRITICAL FAILURE and EXPLODED. I have pictures of my injuries and have retained the original product and packaging. I am requesting an IMMEDIATE response and reply on Monday 12/8/25 from both the manufacturer and CPSC and be provided with a direct contact case worker though this process. Thank you. Sincerely, David Thelen; Concerned USA Citizen

I hereby request a recall of all Manufacturers products containing the Hardware Kit included in this complaint and that these Hardware Kits be replaced and reingineered with a material that will not critically fail and injure another consumer. The product, in the report: Bemis Toilet Seat PN 148SLOWA

    Bemis toilet seat #148SLOWA

I have had two of these seats break at the hinge within two years of installation. I’m reporting it because it is possible that if the hinge breaks while someone is seated there is a fall potential. This happened to me but in this case I caught myself against the bathroom wall. The second one broke when I was lifting the seat but had I been seated I could have fallen into our bath tub and hit my head.

I’m an engineer with 35 years of industry experience and I can see the flaw in the design. The back of the seat is entirely supported by the plastic hinge so eventually it breaks. A simple design change to the existing parts could provide a bumper for the seat to rest against the rim.

    The product, in the report: NextStep® Built-in Potty Seat™, Whisper•Close® with Easy•Clean & Change™ Hinges and STA-TITE http://www.toiletseats.com/model-family/view/nextstep-built-in-potty-seat-whisper-close-with-easy-clean-change-hinges-and-statite/

I have a VERY signifigant safety complaint about your NextStep toilet seat. I purchased one from my local Home Depot around 6 months ago. The magnet that is used to hold the seat up FELL OFF the seat. This magnet poses a signifigant choking hazard. Also, if it’s swallowed with another magnet, a signifigant stomach/intestine perferation hazard. I did not notice that the magnet was not molded into the lid and it sits freely on the exterior of the lid. If I had noticed that, I never would have purchased this product. The magnet is gone, and I can’t find it anywhere. My hope is that it was flushed. But now, I’ll be spending tons of time ripping my kids bathroom apart and digging through toyboxes to make sure it’s not in there. POOR DESIGN. DISSAPOINTED CUSTOMER. My friends have an older model of this seat that had metal hinges and I think that the magnet was embedded in the lid and not able to be separated. MUCH SAFER DESIGN FOR KIDS!!!! I’d like to get this broken seat replaced under warranty, but only if you still offer that design. Otherwise, I’ll find something else.

One other small complaint - the way the child seat is attached, it’s very easy for kids to get thier thighs pinched between the adult and child seat. My son has done this twice and was left with significant brusing.
