  1. No one hurt

Made Sep 7, 2011, published by CPSC Oct 7, 2011

The product, in the report: TechLite Lumen Master, CREE XPE L.E.D., Imported by Costco Wholesale Corporation, Seattle, WA (ITM./ART.564808) Three LED flashlights and a total of 9 Duracell AAA batteries (99543760 Expiration March 2017) (3 batteries for each of the 3 flashlights).

On September 6, 2011 I purchased from Costco in Issaquah, WA a single package containing 3 battery operated LED flashlights. The package states: - Manufacturer (printed on package): TechLite Lumen Master, CREE XPE L.E.D., Imported by Costco Wholesale Corporation, Seattle, WA - Also printed on package: ITM./ART.564808

The package contained three LED flashlights and a total of 9 Duracell AAA batteries (99543760 Expiration March 2017) (3 batteries for each of the 3 flashlights). The package was securely sealed (plastic ‘bubble’ and cardboard) and showed no signs of tampering or having been opened.

Carefully following the included printed directions I placed each battery correctly as shown both on the printed directions and the guide labeling on the battery holder in each of the 3 flashlights and assembled the flashlights completely following all printed directions.

On two of the three flashlights I tested their operation and found no problems. On the third I noticed the outside metal body of the flashlight seemed warm. I carefully opened this flashlight and took out the battery holder which was extremely hot to the touch and quickly removed one of the batteries. While the battery holder’s metal parts were still very warm they began to cool down immediately. I again carefully, following the printed directions and the guide labeling on the battery holder, removed and correctly replaced all batteries and as soon as the third and last battery was put into the battery holder the metal parts and the batteries themselves began to heat up instantly to an extremely high temperature. Again I quickly removed the batteries and the assembly began to cool down.
