The product, in the report: Boruit headlamp goes on head. 3 lights. Rechargeable batteries with plug in to charge
BORUIT RJ-3000 Headlamp Rechargeable LED Head Lamp We had a rechargeable headlight explode last night. It was charging. We had just gone to bed when we heard the first pffffttt. I walked into the hallway when there was a huge boom and something hit the door. We opened the door to this! The room was full of acrid smoke. The entire floor is covered in bits of the batteries I think we dodged a bullet.

The product, in the report: Boruit Headlamp Black. RJ 3000 model head worn flashlight
Boruit Headlamp Was sitting on our kitchen counter, charging, when it exploded like a bomb. One battery flew out and hit a wall. Scorching the wall, bouncing down, scorching the floor, and somehow changing direction and traveling another twenty feet to our front door. It payed on the floor and burned a hole in our flloring. The headset with one battery left burst into flames, damaging our brand new quartz countertop we had installed last month, and sending battery juice and soot into our new tile and grout back splash we had installed in May. If someone would have been wearing that when it exploded, it would have killed the wearer. It lit up our kitchen and knocked items off our counter and across the room with a huge force. This just happened 8am on Friday June 22, 2018. [REDACTED]

Hurt, no treatment Made Jan 29, 2017, sent to the company Feb 15, 2017, published by CPSC Mar 9, 2017
The product, in the report: Boruit headlamp with rechargeable batteries
On Jan 18 my husband removed two Boruit 18650 batteries (4000 mAh rechargeable Li-ion 3.7 V ) from charger and placed them on the counter. Aprrox. one hour later my son awoke and placed the batteries in the Boruit Headlamp Model RJ-3000. I required him to use the restroom before putting the headlamp on. Within less than 30 seconds one of the batteries exploded striking me standing 8 feet away in my left breast. Shortly thereafter the second battery exploded. Smoke filled the kitchen and debris was scattered everywhere. There is no doubt in my mind with the force of the explosion my son would have been instantly killed had he been wearing it.
