How badly anyone was hurt

  • No one hurt 2 CPSC: Incident, No Injury

As each report says it, in short words; CPSC’s own words are under each.

The reports, as CPSC published them

Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

  1. No one hurt

Made Oct 7, 2016, sent to the company Oct 18, 2016, published by CPSC Nov 8, 2016

The product, in the report: Zilotek LED T8 Tubes Daylight strip on the bulb says is. Self Ballasted LED Tube lamp Model 353-2231 Intertek 4009239

Zilotek 48 inch Linear Led Tube purchased at Menard’s.

Plastic casing caught on fire in light fixture, if I had not been around to see the smoke and flames since it was a ceiling light it could of been very serious.

Tried reporting it to Menards, there Chuck Ochs, pretty much even after I sent them a picture said it was not their concern, never had a complaint, and keeps wanting me to return it. He told me it was in a correct fixture, so just return it.

Checked Menards and they have discontinued that brand, after stocking them for a short time.

  2. No one hurt

Made Feb 24, 2012, sent to the company Mar 5, 2012, published by CPSC Mar 19, 2012

The product, in the report: 21 inch ultra thin Zilotek T4 fluorescent under cabinet light. UL listed Portable Cabinet Light E234360 Made in China.

The fluorescent bulb was flickering. Two hours later I noticed it was out. I was going to change the T4 bulb when I noticed the fixture had got hot enough to melt the plastic bulb cover. The white plastic housing below the bulb was scorched brown. The insulation that covers the two ends of the fluorescent tube was shattered and pieces of it were loose.
