The product, in the report: Utilitech white 6 inch. Switchable down light
Installed led down lights from Lowes approximately 2-3 years ago in my new home. They started to stop working and as I removed them to replace them I found 2 of the 3 with melted assemblies. They were in the ceiling so I would have never known if they caught fire until it was too late.

The product, in the report: Product Name: Utilitech 5-in or 6-in 650-Lumen Switchable Round Dimmable LED Canless Recessed Downlight Model Number: MQTL1219-L65K9027 ETL Certification Number: Intertek 5005786 UPC: 822985543224 Color: White trim with integrated LED module Size/Dimensions: Designed to fit 5-inch or 6-inch ceiling cutouts; approximately 7.2 inches in overall diameter, with a low-profile, canless design Wattage & Lumen Output: 11 watts, 650 lumens Features: Switchable color temperature (likely 2700K to 5000K range) Dimmable with compatible dimmer switches Rated for wet locations (e.g., bathrooms, covered outdoor areas) Canless, ultra-thin housing designed to sit directly in drywall ceiling Integrated junction box with push-in connectors ETL-listed and Energy Star certified Purpose and Use: This product is intended to serve as a ceiling-mounted LED light fixture for general residential indoor lighting. It is marketed as a retrofit solution that can be installed without a traditional recessed can housing, making it suitable for remodeling and new construction applications.
What happened that made you want to report? Over the past several weeks, three recessed LED canless downlights installed in our home have failed in a concerning and potentially dangerous way. The internal mounting screws holding the light element inside the can have detached, causing the lighting element to drop and hang loosely within the fixture. This appears to have caused overheating, and in some cases, the lights have physically fallen out of the ceiling. We’ve heard other screws falling inside multiple units, suggesting this may be a systemic manufacturing defect. Given the pattern and the thermal exposure, we are now worried about a possible fire hazard.
What type of product(s) were being used? The affected product is a Utilitech 5-in or 6-in 650-Lumen Switchable Round Dimmable LED Canless Recessed Downlight, model MQTL1219-L65K9027, ETL certified (Intertek 5005786). These are LED retrofit-style downlights rated for wet locations, purchased and installed as part of a home remodeling project.
Who was using the product(s)? The lights are installed in the ceilings throughout our home and used by all household members, including adults and small children. They are part of our regular lighting system and are used daily.
How were the product(s) being used? The lights were being used as intended: installed in the ceiling and connected to standard household dimmer switches. They were used for typical residential indoor lightingturned on and off as part of normal daily routines.
Were there any injuries that happened or may have happened while using the product? Fortunately, there have been no physical injuries yet, but there is clear risk. One light nearly fell out of the ceiling completely, which could have hit someone underneath. More concerning is the potential fire hazard due to apparent overheating and improper mechanical retention of components. The loose elements could create contact issues or expose electrical components, raising serious safety concerns.

No one hurt Made Jul 25, 2017, sent to the company Nov 2, 2017, published by CPSC Nov 17, 2017
The product, in the report: Utilitech Pro LED 65W Recessed Retrofit light, #0599032
The Utilitech Pro LED 65W Recessed Retrofit light, #0599032, has friction clips for installation into the canisters that have provisions for the spring attachments. On 6 different occasions a clip has broken allowing the light fixture to all from the ceiling. Fortunately, no one was under any of these lights at the time and no injuries occurred. If a person had been under one, head injury could occur. If a baby was lying under one, it could break a nose, blind, or cause other head trauma. These light were installed in the Summer and Fall of 2016 and the incidents have randomly happened. The metal in the clips simply breaks into two pieces, separating from the section that is attached to the light by a screw. I contacted the number provided on the box and was simply told that any broken light had to be returned to Lowes. Difficulty with this solution, I have to keep waiting for another light to fall, causing possible injury to a family member or a visitor.
