  1. No one hurt

Made Jun 3, 2025, sent to the company Jun 6, 2025, published by CPSC Jun 23, 2025

The product, in the report: Brand: Truly Green Solutions Model: 881215-RW-TM-CC-90CRI Power: 100-277Vac, 60Hz, 15W, 0.14A, WW/NW/CW Country of Manufacture: China Certification: Intertek ETL Listed

FAILURE ANALYSIS REPORT SITUATION On 05/17/2025 at approximately 0720 hours I, Fire Investigator [REDACTED], was requested by DFM [REDACTED] to contact the necessary personnel at the [REDACTED] Mall to investigate a report of multiple lighting failure events. The purpose of the subsequent investigation was to conduct a failure analysis of the involved lights to determine if any further actions should be taken to ensure the safety of the Mall and its occupants.

The location of this investigation took place at [REDACTED], in Colorado Springs.

INCIDENT INFORMATION Multiple events are associated with this lighting issue: [REDACTED] (12/16/2024) [REDACTED] (2/07/2025) [REDACTED] (5/15/2025)

POINTS OF CONTACT [REDACTED] Fire Investigator, Colorado Springs Fire Department, [REDACTED] [REDACTED] General Manager, [REDACTED] Mall, [REDACTED] [REDACTED] Maintenance Supervisor, [REDACTED] Mall, [REDACTED] [REDACTED] [REDACTED], [REDACTED]

BACKGROUND I contacted [REDACTED], who is the general manager of the [REDACTED] Mall. I met with [REDACTED] and his maintenance supervisor, [REDACTED], to gain context of the events, timeline and equipment involved. This meeting took place at approximately 0900-1000 hours on location, at the [REDACTED] Mall.

According to both individuals, lighting within the mall common areas was updated starting in 2017, with installation of the new LED lights being completed in 2018. They stated that in total, they had 57 lights that eventually stopped working, so [REDACTED] contacted [REDACTED] to send out new lights to replace the first batch. The installation of the second batch started in December of 2024. After three light failures, two of which resulted in a CSFD response due to smoke/fire production, they were told by [REDACTED] to disconnect all the second batch of lights, and that they would send out a third batch. [REDACTED] instructions were to connect four of the third batch of lights within a safe area for 30 days to ensure there were no issues with the lights. Both [REDACTED] and [REDACTED] stated that they had no issues with the third batch of lights, only the second. [REDACTED] stated that after this mornings CSFD response to a smoking light, he realized that it was one of the lights from the second batch, that he had failed to disconnect. They both confirmed that no more of the second batch of lights are energized within the mall.

Both confirmed that while the lights had been swapped out by maintenance personnel, they had a certified electrician confirm that the supply voltage was correct, 120v. They stated that the power supplied to all four of the failed lights was from three different electrical distribution panels. [REDACTED] further clarified that two of the electrical distribution panels are supplied by the same transformer, and the third electrical distribution panel was supplied by a second transformer. They both confirmed that there have been no other electrical issues with any other electrified devices connected to those three electrical distribution panels.

[REDACTED] provided a second batch undamaged exemplar light, as well as two damaged during failure events. The lights were photographed and compared to determine potential commonalities in failure.

DEVICES Exemplar Brand: Truly Green Solutions Model: 881215-RW-TM-CC-90CRI Power: 100-277Vac, 60Hz, 15W, 0.14A, WW/NW/CW Country of Manufacture: China Certification: Intertek ETL Listed Damaged Light #1 Brand: Truly Green Solutions Model: 881215-RW-TM-CC-90CRI Power: 100-277Vac, 60Hz, 15W, 0.14A, WW/NW/CW Country of Manufacture: China Certification: Intertek ETL Listed Damaged Light #2 Brand: Truly Green Solutions Model: 881215-RW-TM-CC-90CRI Power: 100-277Vac, 60Hz, 15W, 0.14A, WW/NW/CW Country of Manufacture: China Certification: Intertek ETL Listed

    EXAMINATION

All lights are circular, white in color, with a toggle switch on the rear to control the light color emitted from the LEDs. All three have two wires leading from the main circuit board, out of the light, to be connected to the power supply. The lights bases are connected to their covers with eight small screws.

The exemplar light base was removed from the cover, with the interior components photographed and utilized as a comparison against the two damaged lights. Upon examination, the exemplar displayed heat damage to the base of the light upon which it was mounted. The heat damage displayed was discoloration of the white base as well as multiple electrical components on the main circuit board.

Both damaged lights displayed melting and charring of the plastic bases as well as their covers. In addition, heat damage was present on the main circuit board of each, as seen while viewing the bottom of the base. The area of origin for both were approximately 3.5-4 inches from the toggle switch controlling the light color. This location correlates with the heat damage noted on the exemplar light.

RECALLS No recalls were located, either past or present, for the appliance involved.

    ANALYSIS/RECOMMENDATIONS

The general location of similar fire effects of both the fire damaged lights, as well as the heat effects located on the exemplar, indicate the failure across all three of the lights viewed was consistent with a main circuit board failure. The exemplar light heat effects indicate a gradual heating event, not a momentary event. Considering all three lights were of the same model and type, I recommend a recall investigation be conducted by the appropriate agency. The Mall staff has confirmed that all lights of this model have been removed.
