The product, in the report: Cadence - 31.5” 70W 1 LED 3-Tier Pendant Chandelier Quoizel Lighting Item: PCCA2831EK
During installation of Quoizel Lighting (PCCA2831EK - Cadence - 31.5” 70W 1 LED 3-Tier Pendant Chandelier), the fixture can be temporarily attached to the mount on the electrical box using a wire with a washer (“stop button”). Per the instructions, this should be done to hold the fixture in place allowing you to use your hands to complete the electrical connections with the wires in the electrical box. In my case, the stop button wire failed and the entire fixture came down. I caught it, but it increased the risk of falling off the ladder or hitting someone below. The wire failed at its connection point to the fixture (not to the mount/electric box).

First aid Made Jun 28, 2017, sent to the company Aug 24, 2017, published by CPSC Sep 11, 2017
The product, in the report: Quoizel dropped pendant item #DW2817C with drum shade and diffuser chrome finish with linen shade.
My Quoizel Downtown pendant lights are hanging over my island and one over my dining room table. My son stood up and hit the edge with his head, with minimal impact the diffuser exploded and shattered into dagger like pieces showering down onto him and my mother. My mother sustained multiple lacerations. When I contacted Quoizel asking why it was not tempered glass, and told them what had happened, they responded with an apology. However they want me to pay for a new diffuser.

No one hurt Made Apr 5, 2014, sent to the company Apr 14, 2014, published by CPSC Apr 28, 2014
The product, in the report: Quoizel flush mount ceiling lighting fixture, model # ML183Z
On the afternoon of March 31, 2014, I was working in my home office when I heard what sounded like a loud explosion. When I turned around I realized that the ceiling light fixture had burst, scattering shards of glass in a roughly circular area beneath the fixture. No one was touching the fixture at the time and, luckily, none of the shards struck me. The light bulbs were 60 Watt [REDACTED] bulbs and were not damaged. It had been several months since the last one had been changed.
The fixture is a Quoizel model # ML183Z, with a label stating it was manufactured in China in July 2006. We did not purchase the fixture; we assume it was purchased and installed by the prior owner who was a contractor who had renovated the property.
I have taken photographs of the scattered shards, the base of the fixture, and the finial which had also burst apart, and would be happy to provide them to you.
Obviously I am very lucky that none of the shards struck me. I am not making any sort of a claim but I respectfully request that you investigate and determine whether this incident is the result of a defect which could affect other owners of this type of fixture. I have collected the shards and would be happy to turn them over to you. We have other Quoizel fixtures in the house which are identical to this one, and want to know if we need to worry about similar incidents occurring with them.

No one hurt Made Jul 1, 2011, published by CPSC Jul 25, 2011
The product, in the report: ceiling light fixture
I purchased a Quoizel ceiling light fixture last year at a closeout sale. Twice the glass globe on the fixture fell down on my bed. I went and purchased a new fixture for obvious safety reasons. To my shock, when examining the Quoizel fixture it showed that that the insulation in the fixture was actually burning and some aluminum sheet insulation was actually melting on a light bulb. the fixture advises no larger than 60W, but I only had 40W bulbs. The glass globe falling could have created a serious injury had someone been sleeping at the time. However, the burning insulation could have resulted in a fire.
