Hurt, no treatment Made Jul 2, 2022, sent to the company Jul 6, 2022, published by CPSC Jul 20, 2022
The product, in the report: Hayden Hardwired Double Tube Sconce in Satin Nickel (item #5655047)
We purchased the the Hayden Hardwired Double Tube Sconce in Satin Nickel (item #5655047) from Pottery Barn. Unfortunately we had a serious quality issue, which I’m bringing to your attention. Without warning, one of the glass tubes suddenly fell out of the light, striking our sink and shattering everywhere. I was in the bathroom at the time and sustained minor cuts (very minor). As I investigated the cause, it appears the light separated from the metal housing. More specifically, I could see the metal housing was still screwed into the fixture itself; the glass simply separated from it. This happened the first time on April 10, 2021 to one of the two lights we purchased. Suddenly yesterday, the exact same thing happened to the other light we had purchased. Since the same defect has now occurred twice, I’m bringing this to your attention.

No one hurt Made Aug 23, 2013, sent to the company Sep 3, 2013, published by CPSC Sep 17, 2013
The product, in the report: Pottery Barn Bernard Pendant
The light was bought for a new house, but was never used. I sold that house and moved in april, and the light was installed recently. The globe crashed to the floor 1-2 weeks ago. It was assumed this happened because the globe was not screwed in properly. At that time, my husband and I got a chair and made sure it was screwed in properly. This morning, the globe was found broken on the kitchen floor a few feet from under the fixture where it had rolled. there were bits of glass on the floor.
