The product, in the report: Portfolio 7.75-in x 9-in Cone Lunar Ash Pendant Light Shade with 2-1/4-in fitter
I purchased 3-Portfolio 7.75-in x 9-in Cone Lunar Ash Pendant Light Shade with 2-1/4-in fitter from Lowe’s in McCandless, PA (ordered online July 8, picked up at store July 12). The shades were installed properly. We arrived home in the evening of July 17 to find one of the shades broke and shattered all over the kitchen counter and floor. I see now that there have been two reports on the Lowe’s website of this same light shade cracking for no apparent reason. I believe this shade should be removed from inventory before someone gets injured. If anyone had been sitting at our counter or standing anywhere in the vicinity, they would have been seriously injured.

No incident, no one hurt Made Jul 8, 2021, sent to the company Jul 12, 2021, published by CPSC Jul 26, 2021
The product, in the report: The fixture that broke and fell from our ceiling was “Portfolio Lola 31-in 9-Light Golden Bronze Standard Chandelier” sold exclusively by Lowe’s as Item #240822, Model #34330 (https://www.lowes.com/pd/Portfolio-Lola-31-in-9-Light-Golden-Bronze-Standard-Chandelier/1238361). There is an existing recall on this model number. However, Lowe’s is STILL selling this fixture (either exact fixture or perhaps they believe it was updated) as “Portfolio Linkhorn 9-Light Iron Stone Vintage Chandelier” Item #442228, Model #34651. This “new” model should also be recalled, as you can see from the reviews that people have been reporting that this one also breaks and falls for at least the last 5 years (https://www.lowes.com/pd/Portfolio-Linkhorn-9-Light-Iron-Stone-Vintage-Scavo-Glass-Chandelier/4755201). NOBODY should install these fixtures in their homes--someone is going to get seriously hurt! The loop that broke on ours certainly looks more plastic than metal, and this is what they use to hold the full weight of a 34-lb chandelier.
The loop attached to the screw nut at the ceiling, which apparently was holding the full weight of the 34 lb chandelier, suddenly broke, causing the chain and the ground wire to also snap and the chandelier to hang from our 15’ foyer ceiling by only the electrical wire. We were forced to cut the power and snip the wire with a tree pruner to prevent the fixture from crashing down onto our banister and tile foyer floor (this was done very precariously as my husband had to stand on the stairs and basically catch the 34-lb chandelier). We are now forced to build a sturdy platform from which an electrician can survey any damage to the electrical box in the ceiling (which may have been caused by the full weight of the chandelier yanking the electrical wire 10-12” further than it had been), and to hang a new, better quality, light fixture. The fixture was the “Portfolio Lola 31-in 9-Light Golden Bronze Standard Chandelier” Item #240822, Model #34330. The fixture was purchased new from Lowe’s when we built our home in 2008 and was installed on our foyer ceiling by a certified electrician. Thankfully, nobody was under the fixture when it broke free from the ceiling, and somehow the electrical wire was strong enough to hold the fixture until we were able to cut it down safely. HIGHLY concerning is the fact that Lowe’s is still selling the EXACT fixture under a different item number (https://www.lowes.com/pd/Portfolio-Linkhorn-9-Light-Iron-Stone-Vintage-Scavo-Glass-Chandelier/4755201) and people have been posting reviews dating back at least 5 years reporting that this “NEW” model number also breaks and falls from the ceiling!

The product, in the report: Aztec kitchler nine-light chandelier sold at Lowe’s stores under the PORTFOLIO brand name. The chandelier is metal with an olde bronze finish and measure about 31-inches wide and 31-inches high. comes with etched amber glass shades and a decorative faux marble ball in the center. The upper arms of the fixture are single long curves and the lower arms of the fixture are “S” shaped. “Aztec” and model number 34330 are printed on a sticker located inside the ceiling canopy at the top

The product, in the report: Portfolio and Transglobe nine-light chandeliers Lowes- Portfolio Brand- Model # 256047 / UPC: 00736916528528 Trans Globe – Model # 2579 EBG / UPC: 00736916512312 A bronze-pewter colored 9-light chandelier with swirl motifs. The scones are a creamy gold color.
On the evening of Friday June 12, 2020 the chandelier fell out of the vaulted ceiling. A scone, 2 light bulbs, and the decorative nub on the bottom (looks like a fancy curtain rod end) shattered. Exposed wires are now hanging out of the ceiling. The ring that was a part of the ceiling plate snapped. The plate is still firmly attached to the ceiling and part of the snapped ring was found on the floor.
Please not that this is not the ring that was recalled before (where the chandelier meets the chain), that ring was replaced with the hardware provided by the company. The ring that broke is where the chain meets the ceiling plate with it being a single unit welded to the plate.

The product, in the report: 9-light chandelier purchase at Lowe’s late 2006 to early 2007 item #240822, dated 12/2006; made in China
The dining room chandelier fell from the ceiling as a result of the screw collar loop failing. The chandelier landed on my dining room table, leaving several deep scratches. No one was in the immediate area at the time. My son was sitting nearby, however, and is now afraid to sit under or near any light fixtures.

The product, in the report: Track lighting - 1 light on a single base and 6 of them placed on a track.
This is Track lighting: 1 light on a single base and 6 of them placed on a different track.
The light is attached to a fixture by a Pinhole Track Kit and resembles a microphone (design).
Manufacture Tel:: 800-523-5702
The lights have never been serviced in the past. The lights are in a hallway that is not used frequently. (GU-10 120 v, 50 watt and UL Listed)
The lights use a split Reflector halogen bulb.
The fixture separated from the base of the unit There is a base that can adjust and tilt, but the bulb housing separated from the mounting housing. This left the bulb hanging by it’s electrical wire. The factory housing split where it is joined by 4 metal post.
The consumer noticed that 2 or 3 more bulbs, on a different track, are on the same path.
He plans to bring this matter to the retailers attention.
There have not been any injuries and the damage is limited to the bulbs, so far.
Current MFG distributor has responded that the fixture(s) are out of warranty.
I am old school. I presume residential light fixtures don’t have a used by date.

The product, in the report: Portfolio (Aztec) 9-Light Chandelier
The chandelier has been hanging in a stairway for several years. One night it came crashing down, damaging the wooden stair railings, breaking several globes which left many glass splinters in the carpeting. No one was injured, and fortunately there was no electrical short in the ceiling. Upon inspection, the “screw collar loop” to which the chain is attached and supports the entire weight of the fixture, had broken from the ceiling mount. This loop is not a part that receives any sort of wear and tear, and I feel should never break under any normal usage circumstance. No one was near the chandelier, which means it simply broke and fell without cause except for the failure of the mounting loop. There was damage to wooden stair railings. Also, this is very concerning because we have other Portfolio light fixtures in the house and I am concerned about their safety.

The product, in the report: Portfolio and Transglobe nine-light chandeliers. The recalled nine-light chandeliers have a two-tone antique bronze-colored metal frame with antique gold-accents and champagne frosted color glass shades. The six antique gold colored lamp holders at the bottom and three at top have a floral pattern that is repeated on components at the top and bottom of the center lamp stem. Antique bronze-colored ornamental scrolling connects all the elements. The chandeliers were sold at Lowe’s as Portfolio brand with item number 256047 and at other stores as Transglobe item number 2579 EBG. The item numbers are located inside the ceiling canopy of the chandelier.
I had 2 of the Portfolio and Transglobe nine-light chandeliers fall from my 20 ft.ceiling onto our floor.
The first fell about one year prior but at that time I was not able to identify manufacturer.

The product, in the report: The glass shade detached from the fixture when the socket ring holding the shade crumbled into multiple pieces
When dusting the glass “shade”, the socket ring crumbled, causing the glass shade to drop from the fixture. When I checked the remaining socket rings, they broke apart as I unscrewed them. The cross section visible in the broken socket pieces are white with a chrome colored “skin”. Could the heat from the halogen bulbs cause the socket to deteriorate?

The product, in the report: Lowe’s Portfolio Chandelier Aztec Model # 34330 manufactured by Kichler’s Home Center Division
My Portfolio Chandelier (from Kichler Lighting, purchased @ Lowes) fell for the SECOND time! Out of the blue, @ 4:30 a.m. on 4/16/17 the lamp fell. The O-Ring (cast iron) from the lamp which was connected to the ceiling junction box broke, causing the lamp to fall. Lamp was off. This chandelier fell (same exact thing occurred) on 11/23/13. We weren’t home when it happened. We came home to the chanderlier “resting” on our dining room table! At the time I googled info and discovered this lamp model had fallen 6 times (for 6 other people) I read it was due to a defective O-ring issue, which Kichler replaced. It was re-hung by a certified electrician and fell again on 4/16/17

The product, in the report: Portfolio/ Transglobal Nine light fixture
Purchased a Dining Room Lighting Fixture( portfolio/transglobal nine light fixture) i’d say few years back that fell from the ceiling because of a failed loop which I still have in my posession as well as the fixture. It did severe damage to my wood dining set, the table and one chair which I could not replace as it is not sold anywhere as of this time. I purchased a whole new dining set now prior to seeing the recall for said fixture. It happened while we were was watching TV in the adjacent room and scared the heck out of us. Please if anyone reads this check to see you don”t have one of these fixtures, the fastening loop looks to be made from some cheap material its not even metal, it is very dangerous especially because of the weight of the fixture!!!!!

The product, in the report: Portfolio nine light chandelier, item number 256047.
Portfolio Chandelier was installed in home we purchased. It was hanging in two story foyer as designed. One morning my wife noticed it was hanging lower. Upon closer inspection, it was observed that it was hanging only by the electrical supply cord and there was about a foot gap from fixture to chain. Upon inspecting the top of the fixture, I could see the fixture loop was broken and the broken remains of the loop were caught in the cup at top of fixture. I immediately connected chain to main body of fixture and the support chain to reduce risk of falling if electric cord broke. My family of six occupies this home. Had the electric cord not supported the weight of the chandelier, this could have led to significant injury since the fixture loop completely failed.

The product, in the report: PORTFOLIO nine-light Kichler Aztec chandelier, model number 34330
Portfolio Aztec Model 34330 fixture hanging in our foyer. The screw collar loop broke and the chain fell onto the fixture. The fixture was hanging only by the electrical cord. I called and the CSR is sending me a replacement screw collar loop but I wanted to ensure that this issue was documented. When I got online to get contact information, I found that there was an issue with the fixture loop and that a repair kit was available. I am also getting the repair kit and will change that out as well. In the meantime, the fixture is sitting over the railing and on the second level floor.

There is an active recall for a Portfolio Kichler “Aztec” chandelier with the exact same failure. I contacted Kichler who claims this is not their light. The product, in the report: chandelier has nine lights and are made of an antique bronze-colored metal with frosted yellow light shades. The lights are arranged in two tiers with six at the bottom and three at top. The nine lamp holders have a floral pattern, which is repeated on the stem in the middle of the chandelier, at the top of the chandelier body from which the chain extends to the ceiling, and on the bottom part of the chandelier body to which the arms that hold the lower lamps are attached. Two pieces of ornamental scrolling are attached to the top flower-shaped piece and extend down into the area of the top three lights. The chandeliers were sold as Transglobe item 2579 EBG and under the Portfolio brand as Lowes item number 256047
My chandelier came crashing down from my ceiling onto my dining room table on Thanksgiving Day.
The ring at the top of the lighting fixture broke, where the chain attaches to the main part of the fixture.
I entered the # in the plate at the ceiling after we went and took it off the ceiling because the chain and the electric were still attached to the ceiling.
When I filled out the on line registration and it told me my light was not affected, I don’t know if I put the wrong # in, really not sure.
The metal gave way just like on the recall page the ring cracked, we have both pieces.

The product, in the report: Portfolio 6-light bathroom fixture
I had this light fixture (item 73655) installed in my children’s bathroom. Recently one of the glass shades separated from its metal threads and crashed to the vanity counter below. This left tiny shards of glass on our counter, in the sink, on the floor and in my children’s toothbrushes and water cups. I carefully unscrewed the other five glass shades and three more separated from their metal threads. They came off effortlessly, which leads me to believe they were due to fall as well.
I suspect that the heat from the bulbs compromised the glue that held the glass and metal parts together. This is a design flaw at best, a manufacturer’s defect at worst. It is not an installation issue or user error.
I suggest that Lowe’s discontinue this item and any others with similar design. Also, this item should be recalled because of its potential to cause injury. You may note that another reviewer mentioned the same problem.

The product, in the report: Chandelier with 9 lamps supported by a bronze s-shaped leg frames.
The chandelier (Aztec, model 34330) fell to the ground. The loop located at the top end of the chain and near the base cap sheared off and caused this failure. Fortunately, nobody was hurt by this serious accident, and we are very lucky that the failure happened shortly after my wife finished routine cleaning from just underneath the chandelier area. The equipment was purchased at the Lowes store in Stuart, Florida (just south of Indian street) back in November 2006 and since it was place in service we did not observed any signs of straining or yielding that would indicate to us that the chandelier was in imminent mode of failure.

Kichler 9 light chandelier was hanging over a formal dining room table. At 3am, it fell and hit the table and a side chair. The ring by which it was hanging failed, allowing the chandelier to fall. My family regularly sit at that table. It had been there for approximately 5 years.
We are nervous because we also own the same brand/style 5 light and pendulum lights.

This incident involves a dining room chandelier purchased from Lowes. My husband and I came in one afternoon to find part of a metal ring on the dining room table and discovered that it had come from the chandelier which was still hanging from the ceiling. It was hanging only by the electrical wire because the ring at the top of the chandelier had broken. The ring was apparently made of a cheap pot metal and could not support the weight of the chandelier. The replacement part sent to us appears to be of the same material.

The product, in the report: Chandelier
The consumer stated that 2 weeks ago her chandelier in her entrance way fell out of the ceiling after being installed 11 years. The consumer went to the retailer where she purchased the first one and ordered another one.
(8/2012) After having the new one installed by a carpenter. The consumer was coming down her stairs she looked over to the chandelier and noticed that the wires were protruding from the top and had started to loosen up.
(8/2012) The consumer contacted an Independent contractor to come out and remove the chandelier from the ceiling before another incident occurred.
UPDATE: In 1998 I purchased the attached chandelier along with other light fixtures for my home at the Lowe’s in Jackson, MI. The fixture was installed by the electrician who had done all of the wiring in the house. The fixture was in my entrance way until 2009 when the fixture broke and fell to the floor. Me; thinking it was a freak accident went to my local Lowe’s in Scio Twp. MI. hoping to find the same fixture since I really liked the style. I was able to order it through a special order catalogue and in a week or so it came and I had the contractor put it back up in my entrance way. Now here it is 2012 and just recently I noticed that the fixture looked like it dropped, so I took my camera and snapped a few pictures and called the contractor out to remove it because I did not want it to fall again. I now know that the loop hook on the top of the fixture is definitely a design flaw, unfortunately I have paid over $1000.00 for two chandeliers and am now out a fixture once again. I went up to the Lowe’s in Scio Twp. to see if there had been a recall on this chandelier and if they were still selling it, I was told by a manager that they probably didn’t sell it anymore and that I should call the company who made it. I walked over to their special order catalogue and found the same chandelier and pointed out to the manger that they do in fact still sell it. I wrote the number down of the company PortfoBo/BeiAir and called their customer service where I was prompted to leave a message which I did and they have never bothered to return my phone call.

The product, in the report: Surface mounted ceiling light fixture
The owner of a rental home was called to the residence due to an odor of smoke. The source of the smoke was attributed to the furnace.
The owner conducted a safety inspection and found that the electrical insulation on wires in the ceiling light fixtures had deteriorated as a result of heat damage. The insulation was discolored, cracked and decomposed so bare conductors were exposed on the wires connected to the lamp sockets. The lamps were fitted with the proper sized bulbs and were installed and maintained according to the manufacturer’s instructions.
Damage was confined to the lamp fixtures. There were no injuries. The lamp fuxture was purchased at a Lowe’s store during 2010.

Wall mounted light fixture: Portfolio Chalet Spot Light GL19951-FBL-I with 40 Watt RL14 lamp.
We had this lamp mounted on a wall above my son’s bed, and the lamp exploded after being on for approximately 15 minutes. We have had this fixture (and a second in another bedroom) for a few years with no trouble. This lamp had not been turned on for months. All that remains of the bulb is the base, the glass is spread out across the bedroom. This was the correct bulb for the fixture (R14, 40 watt), the one that came with the fixture. There are numerous warnings on the fixture about Risk of Fire, and another warning not to touch the lamp. It is possible that someone has touched the lamp, which could have caused this problem, but this seems extremely dangerous.
