The product, in the report: Fluorescent light fixture for 4 bulbs 4 feet long mounted on ceiling.

When changing fluorescent light bulbs in a light fixture the sockets sparked and began to melt when turned on. This continued till I turned the light fixture off. This happened in multiple different fixtures of the same model. I have replaced the light fixtures but am concerned this is a fire hazard for anyone else who has these fixtures installed.

The product, in the report: SGLL Linear Garage Light LED Indoor 24 in Rectangle Flush Mount or Suspension Mounted PIR Motion Sensor Linear

On or around September 9, 2016, I was installing six 2016SGLL Linear Garage Light LED Indoor Light manufactured by Lithonia Lighting in my garage. The ceiling is about 10’ high, so I decided to use the suspension cables that come with the light to hang the lights. I looped one end of the suspension cable into correct location on the light and lifting the light up by the suspension cable to connect the other cable loop into the ceiling hook when the Cable Ferrule that created the loop that was connected to the light at the time let go and caused the Garage light to fall about nine (9) feet to the floor narrowing missing my wife who was holding the ladder steady for me. On inspecting the suspension cable with the cable loop that is created by the crimping the steel wire into a loop using the cable ferrule, I could see plainly the ferrule broke in two pieces. The cable ferrule that crimps the 1/8” steel cable together is fairly small and the alloy the ferrule was made of seems very brittle. I returned the defective product back to Home Depot where I bought it and purchased another, but of the six (6) lights that I purchased three cable ferrule ends broke apart of twelve (12) cables, I used. I loved the lights, so to fix the problem, I purchased 1/8 in. [REDACTED] and use that to secure the end of the suspension cable that had a cable ferrule.

The product, in the report: Inside LED Lighting. LED Lighting with Square white lamp diffuser. Square backplate with an LED driver and round circle of LED’s and white square plastic cover.

Two seperate fire runs to the same elderly care facility with new lighting within 5 months of each other. Both runs the new LED lighting same model, manufacturer and same installation time, this fixture overheated to the point where it melted plastic around the fixture. We are concerned for the saftery of the residence as they just had a fire not to long ago and these lights were all installed after the fire as part of the remodel. Both times the fixtures have overheated causing “hotsmell” and melted plastic lighting shades around the ficture.

The product, in the report: Flourescent Electrical Light Fixture

On November 21, 2013 I heard a noise coming from one of our spare bedrooms. Upon investigation I opened the closet door in the bedroom and discovered one of the 8 foot fluorescent bulbs from the Lithonia electrical fixture had fallen from the ceiling fixture and was in flames. The closet was full of clothes, pocketbooks, and other personal belongings, most, if not all of which were flammable. I reacted by grabbing the bulb and pulling it free from the closet while slapping the flames out with my hand. As this was happening I noticed flames on our wooden floor in front of the closet. I stomped out those flames. The charred piece of material that I lifted from the floor was later identified as a piece of the Lithonia plastic protective tube guards used to protect these bulbs from accidental contact or breakage from the bulb.

I called 911 and the Clermont, Florida Fire Department responded with two trucks. The firemen were here about 45 minutes and told us that we were extremely lucky that we were home and responded so quickly. They said that our house would have burned to the ground and all of our possessions with it if we were not home or ignored the noise in the spare bedroom.

An electrician installed the Lithonia fixture, model C2 96 120 GEB on the ceiling of the closet approximately six weeks earlier. It was used many times during those six weeks without a problem. It had two fluorescent 8 foot, T12 bulbs and both had Lithonia Clear Tube sleeves/guards, TGT12 CL 8FT R24 around them.

We called in a second licensed electrician to inspect the installation job of the installing electrician. He concluded that there was nothing wrong in how the installing electrician installed or ran the wiring for the fixture.

I contacted Lithonia on December 2, 2013 and reported the incident to [REDACTED] who works in their PMD Post Sales Department. [REDACTED] requested pictures that showed the burned fluorescent bulb, the burned socket bracket and the identification label. We sent him the pictures.

Mr. [REDACTED] subsequently responded to us by email and informed us that, “the likelihood of the lamp sleeve causing the problem is quite high. He further stated in his email, “In general, we do not recommend tube guards with fluorescent lamps that are operated on instant-start electronic ballasts, which this fixture has. There are different types of tube guards, some of which have limited temperature ranges, but they all increase the possibility of poor lamp-to-lamp holder connection.”

Our Lithonia fixture did not display ANY language warning us of potential fire if used with any plastic tube protector or with an instant start electronic ballast referred to by Mr. [REDACTED]. I am attaching both the Lithonia plastic Tube Guard literature, and the warning label on the Lithonia electrical fixture box. As is evident there is no warning in either which addresses the possibility of poor lamp-to-lamp holder connection or fire. If this is really true, Lithonia is NEGLIGENT in not warning consumers and their electricians. Lives could be lost.

I subsequently sent both a letter, and an email to their Vice President in charge of this product, and have received no response. His name is [REDACTED], and he can be reached at Lithonia LIghting, [REDACTED] It is my upmost hope that you will investigate this. If we had been asleep we could have died in the fire. I fear another family may not have our good fortune.

The product, in the report: Lithonia Lighting 2-Light Flush-Mount Steel White Fluorescent Light

On the evening of Sunday, September 15, 2013 my husband was installing a Lithonia light 2-light flush-mount steel white fluorescent light fixture that he purchased from Home depot on August 4, 2013. My husband wore [REDACTED] latex gloves to remove the items from the box and place them on the floor. He was kneeling on the floor next to the items when he went to place his fist on the floor so he could brace himself to review the instructions when he hurt his hand. He miss calculated the closeness of the frame cover and his fist swiped against it. The sharp edge cut through the glove as well as my husband’s skin. He suffered a deep laceration to the back of his hand just above the right 3rd metacarpal. We cleaned the wound and dressed it. His finger was weak but he had a little movement. The next morning, Monday, September 16th his hand felt worse, so we called our primary care physician and he was seen right away. During that visit he was given a tetanus shot and referred to a hand surgeon. We saw the hand surgeon that same afternoon. He put my husband’s hand in a splint and scheduled him for surgery on Wednesday, September 18, 2013. During the surgery it was determined that my husbands’ tendon was completely severed. He will be in a splint for the next five weeks unable to use his right hand, his primary hand. The edge on the cover and mount are razor sharp. I feel the edges could have been sanded if not bent so that they would not be so sharp. This product is dangerous. It does not appear that consumer safety was taken into concern when it came to making this product because the mental was cut and then bent into shape the edges where not finished.
