The product, in the report: Kichler 43848WHLED30T Horizon 6.5” LED 3000K Downlight in White Brand-Kichler Color-White Material-Metal Style-Contemporary Light fixture form-Recessed
We have installed Kichler flush mount LED light in a newly constructed bedroom in our home. After a 6-12 months the light no longer worked. When we took the light down we saw moderate melting of the plastic protecting the LED light strips and melting of the foam gasket on the light. We then inspected the electrical connection and system to ensure it was not an electrical problem and installed another Kichler light (same Model and manufacture date). Another 6 months later and there was smoke and arcing damage on the new light again. The electrical work is new and has been inspected by the State of MN and done according to the Electrical Permit. The breaker never tripped during either of these occurrences so it is my assumption that there is some arcing causing the heat and melting occurring from a design flaw of the screw locations on the light itself. We have since then replaced all lights with this model from our home with another brand of light.

The product, in the report: 6-light linear ceiling fixture
The consumer stated that he noticed over the past three years that the prongs on the bulbs in the fixture were black. Every time they put a bulb in, it goes out.
On 1/27, a burnt smell was coming from the fixture. When the bulb was removed, it had a burnt smell. The bulb in question is a 50-watt halogen. When the consumer contacted the firm, he was advised that he should be using a 7-watt LED bulb. However, the consumer is using the wattage and type of bulb that is included in the instructions.

The product, in the report: Kichler 2347OZ Circolo Chandelier 12-Light, Olde Bronze. The chandelier was installed by the home builder Pardee. It was one of the options when purchasing the house 3 years ago.
Our Kichler Circolo Chandelier installed by our home builder Pardee and contracted out to JAAM builders also known as Scilling came crashing down from a height of approximately 15 ft. We contacted the builder Pardee and the representative stated the Chandelier became unscrewed. Unbeknownst to us, the Chandelier is simply screwed through a rod without any locking mechanism. This means that without any warning and minimal rotation in time it becomes unscrewed and falls. The Chandelier came crashing down and completely shattered and damaged our hardwood floor. Had it fallen on someone there would be serious injuries.

No one hurt Made Nov 21, 2013, sent to the company Dec 2, 2013, published by CPSC Dec 16, 2013
The product, in the report: Kichler Lighting Olympia 15 Light Chandelier in Antique Pewter 1691AP
Our Kichler Olympia 1691AP chandelier fell from the ceiling of our two story foyer and was destroyed on impact, which would have seriously injured or killed anyone underneath it. We phoned the electrician who installed it. He came out the next morning to inspect it and told us the “hickey” (he says that is the real term) was defective. This is the metal piece that screws onto the nipple and connects the chandelier to the chain. The metal fractured in a jagged pattern, and is obviously defective.
After searching on Google we discovered that Kichler has another very similar chandelier that has faulted in what sounds like the same place and manner. Kichler has issued a recall on this chandelier. It is recall number 13-222 and was issued on June 20, 2013. We believe our chandelier has the same defect as the recalled fixture.
The chandelier was hanging from the ceiling of our two story foyer, and has been there since January 2008. We hardly ever used it as the light from it didn’t illuminate our foyer very well. Instead we use installed recessed lighting and two floor lamps. The fixture was connected to a chandelier elevator to make changing bulbs easier. However, since we have misplaced the key to the elevator, it has never been used and placed no stress on the chandelier.
The damage amounted to a complete loss of the fixture, as well as putting dents in our hardwood floor. Anyone who might have been under the chandelier when it fell would have been seriously injured or even killed. My wife and I had passed under the chandelier many times the day it fell, and fell incredibly fortunate that we were in a different room when it broke and dropped.

The product, in the report: Kichler Aztec light chandelier model 34330
We have the Kichler Aztec chandelier shown in the recall. It hangs high in the stairwell of our house. It fell at 4 in the morning about a month ago. As it was up so high, it did not hit the ground. The electrical wires pulled out if the ceiling and were holding it about 15 feet down from the ceiling. The ground wire was broken. Scary at 4 in the morning. We went to Lowes with it, they would not take it back because we had bought it years before. They sold us a piece to fix it and we had to hire an electrician to repair the light and wiring. I didn’t know the brand name of the fixture until seeing the recall.

No one hurt Made Sep 6, 2012, sent to the company Oct 12, 2012, published by CPSC Oct 26, 2012
The product, in the report: Two globe light fixture for bathroom
The consumer says that the fixture was professionally installed in the bathroom. The light was on as the installer worked in the bathroom. He heard a crack. They could not figure out where it was coming from. About an hour later, the lights were still on and one of the globes imploded sending hot glass throughout the room. The bulb (a 100 watt halogen bulb) did not break. The consumer says it was due to the extreme heat from the bulb that imploded the globe. He went back and read reviews of the fixture.
Customers were recommending a dimmer switch because of the intensity of the light. he can only make the assumption that the bulb is being used is too hot for the fixture or there is a defect in the glass of the globe. The consumer says the implosion of the globe could have caused serious injury to anyone nearby. The consumer considers it to be a dangerous fixture for reasons to be determined.
Consumer Added: Spoke with customer service who told me that the globe imploded because the screw holding the globe was overtightened. How could they make that statement without seeing the fixture? I then spoke with the installer who said that was absolutely not the case and he has installed many similar fixtures.
