The product, in the report: ORION ISON LED Retrofit Modular Gourmet (LDRMG1)
Electrical maintenance workers were installing the Orion ISON LDRMG LED retrofit modular light fixtures in one of the office buildings in our facility. Once they replaced the old fixtures with the new Orion ISON fixtures, they resupplied power to the breaker and found that one of the Orion ISON units was not working (not illuminating like the other new Orion ISON fixtures were). One of the electricians opened the light to investigate the cause for it not working, and when the metal of the light fixture touched the metal of the I-Bar in which it was attached, an arc event occurred. Further examination of the unit found that the HOT and Neutral wires had been improperly installed in the factory. I have pictures from the investigation if you’d like to see them.
