The product, in the report: Wall-mounted bathroom light fixture Columbia Lighting (fluorescent fixture) Issue #C 138670

We had a fire that was caused by a malfunctioning light fixture, mounted on a bathroom wall above the mirror of a District 66 grade school. The point of origin was near the receiving contact of the fixture that accepts the light bulb prongs. I had the Facilities manager look up the manufacturer so I could pass the information on to the CPSC. Here is the fixture: * Columbia Lighting (fluorescent fixture) * Issue #C 138670 Not sure if you handle this type of information or if there is another avenue. Any questions, feel free to contact me via email or [REDACTED]. I respond as soon as I can.
