  1. No one hurt

Made Sep 3, 2022, sent to the company Sep 8, 2022, published by CPSC Sep 22, 2022

The product, in the report: This is the 16 in. Bright White Round LED Flush Mount Ceiling Light Fixture 1640 Lumens 4000K 22-Watt Dimmable ENERGY STAR Rated that I purchased from Home Depot. This is the product on their website... https://www.homedepot.com/p/Hampton-Bay-16-in-Bright-White-Round-LED-Flush-Mount-Ceiling-Light-Fixture-1640-Lumens-4000K-22-Watt-Dimmable-ENERGY-STAR-Rated-54075341/206355272#overlay

A LED light fixture suffered an internal failure and burned as it failed. I purchased this light fixture about 3 1/2 years ago and installed it in my kitchen. I am very concerned because the fixture got hot enough to melt and char the plastic. It could have caught my ceiling on fire, but fortunately there were no injuries and no apparent damage to my house.

The product, in the report: Vanity light fixture, 3 lights made by Hampton Bay

Glass globe and ceramic housing with LED light to master bedroom vanity light dropped from fixture into sink scattering the globe. My wife was in the bathroom when this happened. No injuries but when I went to pick up the metal piece which houses the led bulb the item was hot when I went to pick up for discard and had to drop. The ceramic/metal housing which globe is attached to broke allowing globe to drop. Picture is available of the broken ceramic part which held the globe in place plus picture of vanity light is available. I purchased from Home Depot on November 19, 2016. Model item 73996 made by Hampton Bay.

The product, in the report: White 11 inch flush mount with 2 lights ceiling fixture

The submitter is reporting the unit for posing a health hazard and failing to have appropriate warning labels.

He opened the package while it sat on his lap and shortly after he began to feel a strange sensation on his chest/ abdomen area. He said his chest felt itchy and as if someone was poking it. He indicated that when he opened the package he was not wearing a shirt due to the warm weather. He believes the product has fiberglass and that the fiberglass particles touched his chest, causing the adverse health effects.

He relayed his symptoms to his daughter, who is a nurse and she recommended he rinse his chest with Epsom salt and warm water. He also applied cortisone cream. He said after a few days he felt better. He did not seek further medical attention.

He contacted the firm on April 16, 2019 to report the incident and request that they label their product with appropriate warning for fiberglass. He was advised that they could not assist him and referred him to contact the place of purchase.

He then contacted the retailer and reported the incident. He also returned the product and received a refund. He was told that someone with their claims department would reach out to him. He received a call from [REDACTED] (Phone # [REDACTED]) with their claims department on May 15, 2019 and he told the submitter that he would get back in touch with him but the submitter has not heard back.

The submitter believes the firm should label the product to advise consumers of the danger and their failure to do so is negligent.

The product, in the report: Hampton Bay white ceiling fan with lights

Hampton Bay white ceiling fan with lights. In order to change burned out light bulbs, we needed to remove the glass dome that was covering them. This dome is screwed into the fan, and requires turning to remove, instead of being the kind that has screws holding the dome in place. When I went to unscrew the glass dome, it shattered, hitting me in the face with a large chunk of glass that cut my nose tissue and cartilage down to bone. My nose had to be reconstructed by a plastic surgeon. I don’t know why this isn’t made of safety glass, plastic, polycarbonate, etc, but I was severely injured, my face will be scarred for life, and I am very lucky none of the shards went into my eyes or hit my daughter who was nearby. The surgeon said that if the glass had been any heavier, it would have completely severed the tip of my nose.

The product, in the report: Home Depot Nassau 58020, brown leaves, rubbed bronze fan arms,

Home Depot fan Nassau 52 in. Natural Iron Model 58020 had a short in the light receptacle. The light bulb was receiving way too much power and you could see the light as a little too bright. The bulb burned my hands even through gloves. After I waited a rather long time to check the bulb. All bulbs that go in this fan come apart. It causes the bulb to become so hot it disconnects from the base. The fan was upgraded to an LED CFL bulb however the model I have is dangerous. It is a different bulb That uses more power. The fan causes the bulb to get too hot and is dangerous to use.

The product, in the report: Hampton Bay 1-Light White Globe Flush mount with Pull String - Model # JO206H

I installed a Hampton Bay 1-Light White Globe Flush mount with Pull String - Model # JO206H, purchased at Home Depot, in my closet. I screwed in a light bulb and closed the associated Circuit Breaker (CB), to turn on the light. To my surprise, the CB cleared open twice. After removing the fixture and the bulb within, I visually realized the center conductor tab for “hot,” power was bent over by ~ 30 degrees. Apparently this tab was not fastened correctly and twisted over as I installed the bulb, thereby causing a short circuit to the grounded threaded socket for “return.” Fortunately, the CB tripped and cleared the fault, however for older say framed houses whose CB’s are known not to trip upon fault could withstand a possible fire hazard. Photo of damaged device will be appended.

The product, in the report: light fixture to hang on the wall

Socket holding bulb melted, blackened; bulb corroded

Socket was supposed to take up to a 60 w bulb; I had only a 6 or 9 w bulb so I did not overlamp the socket

If I had used the 40 w bulb, supplied with the fixture instead of much lower, it could have started a fire

The wires from the fixture to the outlet box were not burned and did not show any sign of damage or improper installation

I have two other identical sconces, again with reduced wattage bulbs and neither of them has shown any sign of similar blackening/melting
