  1. No one hurt

Made Jan 4, 2022, sent to the company Jan 19, 2022, published by CPSC Feb 2, 2022

The product, in the report: CNSUNWAY 4FT LED Shop Light, 40W 5000K Daylight T8 Integrated LED Tube, V Shape, High Output, 5200LM Super Bright Linkable Light Fixture 4 Foot with Plug for Garage, Workshop, Depot (6 Pack). Looks like they did the amazon bait-and-switch where there used to be a hard-wired electrician-only version for sale, then they switched the product to this one with wall plugs.

I bought these LED shop lights from Amazon: https://www.amazon.com/dp/B07MQLG364 Noticed they have male power plugs on both ends. I verified it with a voltmeter. Plug one end into the wall using the supplied cord, stick your finger in the other end, get electrocuted. Worse, if you plug another of the supplied cords in the other end, now the blades of that plug are electrified.
