The product, in the report: 60”, 5 blade, ceiling fan with single light

7/13/2021: The ceiling fan was in the off position, but the light was on for approximately 30 – 60 minutes. The consumer and her husband was sitting in their living room, when the glass cover over the bulbs exploded. Glass flew across the room and into the kitchen.

The glass cover is approximately 8” across in diameter. This is the original cover that came with the fan.

7/14/2021: The consumer called the manufacturer and reported the issue to their Customer Service Dept: Tel: 800-986-3460. After the consumer reported the problem, their rep. (name unavailable) offered to send a new light cover for her to install. The consumer no longer feels safe with the fan and declined the offer for the replacement bulb.

There were no injuries and no damage outside of the bulb.

UL60-CANS, was also written on the product, but the consumer not sure what that number represents.
