The product, in the report: 48” Linear LED T-8 Tubes, Replacement LED tubes for florescent bulbs.

I installed 18 of these 48” Linear LED T8 Tubes in my garage in 3 approved 6 bulb fixtures. Within 1 month one bulb quit working. When removed I noticed it was deformed on the back side of the bulb housing like it had been hot and blistered the plastic. I returned the bulb to Menards where I purchased them and was given a replacement bulb of the same type. Within the next 2 months another bulb quit working, I replaced it. It had not blistered like the first one. Last week, Within 5 months of purchase(4/20/16) a third bulb quit working. This time I had just entered my garage and turned on the lights. I was in the garage for about 30 seconds and heard a sizzling sound and then started to smell something hot. I looked up at my lights and saw smoke coming from a light fixture and molten plastic dripping onto the floor. I quickly shut the lights off and removed the bulb that had quit working and shorted out and melted. I am glad that I was present and recognized the problem quick enough to prevent a fire or further damage. The failed bulb blackened and damaged the light fixture reflector. I have attached many photos of both bulbs. When I took the first bulb back I voiced my concern and was told they had no other problems with the product. I have repeatedly tried to contact the distributor (American Lighting, Inc.) Their customer service keeps sending my calls to a “[REDACTED]” who will not return my calls.
