The product, in the report: Adjustable white 5-in or 6-in 650-Lumen Soft White Round Dimmable LED Recessed Downlight
An LED retrofit downlight bulb/fixture made by Utilitech and sold at Lowe’s overheated, partially melted, and fell out of a recessed fixture about 10ft to the floor almost hitting someone nearby. No one was hit or hurt, not even burned when picking up the very hot object, though all were a close call. The 6 pack of this is listed on Lowe’s site at https://www.lowes.com/pd/Utilitech-6-Pack-Integrated-LED-5-in-or-6-in-65-Watt-EQ-White-Round-Dimmable-Recessed-DownLight/1001767814 If you follow that link, click on the reviews, and filter for lowest star rating to highest, you’ll see that for some time now, many customers have been experiencing this same issue or similar, showing a clear pattern of manufacturing defects, probably in the LED Driver, causing a clear hazard of fire, burn, or at least blunt force trauma to persons beneath the fixture should it overheat and fall.

No one hurt Made Sep 21, 2022, sent to the company Sep 23, 2022, published by CPSC Oct 7, 2022
The product, in the report: White enclosed/contained LED household light bulb for recessed light fixtures. Utilitech Pro, E26 2700K, 17.5W, 120V, 60Hz, 158mA, 1100luments, UL, LED LAMP E359760, Listed 0115 (“Suitable for damp locations”) (NOTE: Bulb was installed indoors, not in a damp location)
A “Utilitech Pro” LED lamp bulb (model E359760, 17.5W, 120V, 60Hz, 158mA, 1100 lumens. UL) installed in a kitchen ceiling recessed can light caught fire while on and burned through the bulb housing causing smoke and burn damage to the the light fixture and surrounding ceiling. Fire was extinguished after turning the light fixture (switch) off with no further damage. The light “flickered” momentarily before making a popping noise and then combusting. The bulb had been installed for between 9-12 months. Original purchase was from either [REDACTED] or Amazon (uncertain which). Attached photos are of affected bulb side by side with an unaffected bulb from the same purchase which was installed in a recessed fixture next to the one which caught on fire.

A can Utilitech Pro LED bulb failed and melted the housing at the base of the bulb. We were home at the time and witnessed the lightbulb go out and begin to smoke. An inspection of all of the canned lights in our kitchen (11) revealed two different types of Utilitech bulb, one that had a two piece construction and was heavier, and one that was lighter and more unibody. 9 of the 11 were of the same type that failed, and all exhibited what appeared to be heat strain (black smudges at the base of the bulbs), where 2 of the 11 were the other type and appeared undamaged.

The product, in the report: 6 inch LED recessed light made by Utilitech. Model : DLS24-06C27D1E-WH-F1 MFG Date 1015. The back of the light has another identifier where the fire occurred that reads ‘dimmable constant current LED Driver’ Model:JWD 20B1
A recessed LED light inside my home in a completely dry location (Utilitech Model: DLS24-06C27D1E-WH-F1, MFG DATE:1015) flickered out and caught fire inside the recessed ceiling frame/holder. I immediately removed it from the ceiling after turning the light switch off. The back of the light has a 1 1/2 inch melted hole in it, and it charred the metal light socket. I am convinced the fire would have spread if I did not happen to witness this directly. There weren’t any injuries, but I am concerned about the other 80-100 lights in my home.

The product, in the report: Utilitech led light bulb model L18TM4/5K
I had a Utilitech led light bulb melt while in a table lamp. This lamp holds 2 bulbs and I had the same model of bulb in both spots I smelled an electrical smell and 2 days later found that this bulb had melted and exposed the interior of the base of the bulb.
The bulb was held in an horizontal fashion and it looks like the weight of the white glass pulled open the base when it heated up to what must have been a very high temperature. I believe I was fortunate that it did not cause a fire.
This bulb is identified as having mercury inside of it and I am concerned that I was exposed to mercury when this bulb failed
I have the bulb and I have pictures of the bulb I can send

The product, in the report: Utilitech 9.8W LED Lamp
Today I had a Utilitech brand 9.8W LED bulb (PN: LA19DM/800/LEDG2 UL# E330072) fail with a glowing piece of circuitry landing on my dining room table burning the wood of the table. This bulb and three other just like it have been in my fixture for a few years. The fixture has the bulbs facing down in an open area under frosted glass diffusers. The failure caused the circuitry to overheat and the plastic part attached to the base fail. This allowed the bulb and heat sink to fall onto the table with a glowing red part landing on the table. If I had not been there able to move the piece to the sink using a wet cloth or there had been papers under the bulb this could have caused a house fire.
I have taken pictures of the remains that I can provide electronically and I have the parts in a bag to prevent the stink from causing my coworker from thinking I damaged one of the devices we are developing.

The utilitech pro lightbulb was installed in my overhead can lighting in my kitchen. The canned lighting consists of four bulbs total. While I was away from the house, the utilitech pro lightbulb burned it self up. My circuit breaker did catch it luckily, or likely it would’ve caused a fire. My house smells of burnt plastic.

The product, in the report: Box of 2, 85w, soft white, 15.5w LED, BR40 Flood Lights
Purchased a box of 2 Utilitech 85w Soft White LED BR40 Flood light Bulbs, item # 0441164, at Lowes about a year ago and threw away the receipt after I used it on my Kitchen cam light. Lowes owns Utilitech. The other bulb is still in the box I still have at home. On or about 2/27/18 the bulb blew up in a white flash on my ceiling and melted, leaving a smoke stain. I was present when it happened. The bulb had melted in the cam socket. I still have the bulb and pictures of the damage. I had to hire an electrician because I was concerned that the cam was defective. They told me the cam was fine but that the bulb was defective. I called Lowes customer care in an attempt to get reimbursed for the electrician as well as the bulb and they told me to go to the store. I went to the store and they told me their system had no record of the item. They told to call Utilitech and gave me their number. I called Utilitech and they put me through to Lowes customer care again. Customer care said that someone would call me back and no one ever did. I got busy and with other things which is why I am following up with this e-mail today. At the minimum if Lowes doesn’t realize it, this bulb is a fire hazard and I nearly had one in my home. By the way, I went online and downloaded a picture of the same item that was once sold at Lowes. It seem it may have been discontinued.

The product, in the report: T8 LED replacement for fluorescent tube
Light fixture in my kitchen caught fire. This was an LED replacement for a standard 4’ fluorescent tube. The light had been in operation for about three months without incident. This morning the tube caught fire near one end, broke in half and dropped down onto the light diffuser grate, melting the grate and causing molten burning plastic to drop onto the kitchen floor. I called the fire department, but was able to put out the fire before they arrived.

The product, in the report: CFL Light bulb
I had a Utilitech 13W CFL Light Fixture Light Bulb in a light fixture with three other bulbs of the same kind. I heard a slight popping sound and realized the light had burned out. I didn’t think much about it at the time. Figured I’d replace it later. Then, about 5 minutes later, I smelled burning plastic. I looked at the light fixture and the light bulb base was melting. So, I turned off the light fixture and waited a few moments and carefully removed the bulb.
My big concern is that if I had left the light on while I was out of the house and this would have happened, I believe the house would have burned down.

The product, in the report: This is an LED bulb; it looks like those recalled.
Utilitech LED Light Bulb was in an enclosed light fixture in hallway. It caught fire and melted. Luckily the smoke detectors went off and we cut the power source. There was soot and black smoke. The base of the bulb was melted and the part that screws in to the socket detached and stayed in the socket when we tried to remove it.

No one hurt Made Nov 27, 2017, sent to the company Dec 7, 2017, published by CPSC Dec 21, 2017
The product, in the report: Standard size (A19) LED light bulb, Utilitech brand (Lowe’s store brand), warm or soft white spectrum.
The evening of 11/24/2017 while myself, my wife, and my in-laws were playing cards at our kitchen table, my wife pointed at one of our can lights and said “fire”. We all looked and observed several flashes followed by a cloud of smoke which registered from the bulb and spread out across the ceiling. The other 7 can lights which shared the circuit with the defective bulb flickered a little bit during the burn-out of the problematic one, but did not go out (i.e. the circuit breaker did not trip). We turned off the switch to the lights and watched to ensure no fire spread. The can which housed the bulb was blackened, but no further flame was observed once the bulb itself had stopped burning. The catastrophic burn-out of the bulb took place over a period of about 5-10 seconds. I took a couple pictures of the burnt out bulb in place, and then with the power off, I retrieved a ladder and using a silicone oven mitt I removed the bulb and cleaned up the receptacle as best I could. I took pictures of the burnt out bulb, and removed all bulbs which were of the same type from our home.
The light bulb was a Utilitech LED, model LA19DM/LEDG2. 7W, 79mA, 450 Lumen, UL#E330072. The flame+smoke exit point is observed to be a hole approximately 1/8 inch in diameter in the plastic at the very base of the bulb, where it transitions from the metal which screws into the socket into the plastic housing of the bulb itself. The glass portion of the bulb is intact.

The product, in the report: Utilitech 2-Pack 60W Equivalent Dimmable Soft White A19 LED Decorative Light Bulb 8-watt (60-watt equivalent) soft white A19 filament bulb Rated operational lifetime of 10,000 hours Light output of 800 lumens Dimmable, customizable levels of brightness to set your desired ambient lighting mood Unique strand-style LED arrangement provides a classic replica of antique light bulbs 2700K color temperature, soft white appearance Suitable for damp locations, indoor or covered outdoor use
I purchased 4 Utilitech All Glass LED A19 bulbs Model # YGA16A02-A19-FR-8W in December 2016. They were listed as ok for damp areas. Listed Not OK for totally enclosed fixtures (mine weren’t enclosed). Contractor installed them in January in a 4-bulb bathroom vanity light. One bulb exploded on his head a few hours later but the inner bulb remained. Attributed it to being a faulty bulb. A couple months later, heard a pop in the bathroom and saw another bulb exploded and the light was not even turned on (inner bulb exploded also and the previously intact inner bulb on the original bulb had exploded too). What was really concerning is that the filaments or whatever it is inside the bulbs continues to light when turned on. Took all 4 bulbs back to my Lowes store and was told there was nothing they could do about it unless I wanted to open an incident with the Utilitech company. The Utilitech company’s website is blocked as unsafe by my Norton Antivirus software.

The product, in the report: Utilitech Pro LED Light Bulb - LA19DM/800/LEDG2 9.8W, 120VAC, 60Hz, 89mA, 800 Lumens, UL#E330072
Purchased an Utilitech LED light bulb from Lowe’s approximately a year ago. Trying to convert my house over to LED bulbs for energy usage. I bought a total of 15 bulbs that day. 2 of the bulbs were put into a Lowe’s ceiling light assembly with glass bowl covering the bulbs.
On late Sunday night/early Monday morning (12:35 a.m.), June 28th, 2016 one of the bulbs caught on fire while the light was on. You could see flames flickering inside the bowl until it filled up with black smoke and ash particles. I turned off the power to the light and the fire went out before having to do any secondary extinguishing.
After approximately 15 minutes, I took apart the light assembly and removed it from the ceiling to verify that no damage had been done to any wiring or the ceiling. Wiring and ceiling were OK.
One of the bulbs was melted just above the socket area. No other damage was noticed except for smoke and ash damage from the burning.
I have went online to try and find information on the product, but it is a Lowe’s specific product. Currently working with a Lowe’s manager at my local store to get replaced and escalate the issue.

CFL Utilitech 18W 1100 Series light bulb used in my bathroom ceiling burnt into scorching and blackening the metal plate above the light. It could have caused a fire if it had got past the metal plate. The light fixture had a plastic cover located just under light bulb that was unharmed. The light was used in a very small bathroom with shower, commode, and sink. The light was not to be used where it would come in contact with water. Box said not to use where directly exposed to weather.

The product, in the report: LED LIGHT BULB Utilitech Pro 9.8-Watt (60W Equivalent) A19 Medium Base {E-26) Warm White Dimmable LED Bulb. “I do not have the original packaging. Attached in this report is a photo of the product on the store shelf, copy of the purchase receipt, printed description of the product from lowes website, and photos of the product after the incident occurred.
On March 8, 2014, I purchased and installed eleven (11) Utilitech LED bulbs from Lowes. I used these bulbs to replace ten ( 10) 60W incandescent light bulbs in the kitchen. Four (4) of the bulbs were installed in the ceiling fan in the center of kitchen and six (6) in an over head hanging light over the kitchen table. These light are used periodically and are never left on overnight or used for any extended period of time. On March 11, 20141 was working in the garage directly off the kitchen area. I had turned on the ceiling fan light prior to going out to the garage. The ceiling fan was not on. After about twenty (20) minutes I came back into the kitchen and at that time I heard a very loud cracking sound coming from the ceiling fan light. I looked up and saw black smoke for- 1 second and then flames shot out from one of the lights. I immediately ran to the garage for the fire extinguisher, shutting off power to the light on my way out. Within seconds I came back in with the fire extinguisher but there were no visible flames only smoke. I watched the light for about fifteen (15) minutes and thought it was safe to remove the bulb from the socket. The top of the bulb was cool to the touch but the base of the light bulb was extremely hot. I placed the bulb out in the garage to cool. About twenty (20) minutes later I checked the bulb and the base was still warm to the touch. l removed all these LED light bulbs I had installed immediately. The ceiling fan was installed when this house was built in March 2012. I stopped at the Lowes store that I had purchased the bulbs from got a copy of the receipt and told them what happened and that I would report this incident to the CPSC and provide them with a copy of the report.

The product, in the report: LED 60w replacement light bulb
Installed LED light bulb. After about two hours of constant use, it started to burn. On the bulb, it says “suitable for use in enclosed illuminaries” ours was an enclosed light fixture. My wife heard a popping noise and went to investigate. The ceiling was full of grey and black smoke. If we had not been home, our house would have burned down. This is a fire hazard.

The product, in the report: CFL Bulb, Utilitech Brand. L23TM6/27K(N) UL E170906
In my kitchen, I have a lamp above the island. There are three bulbs in that lamp. My wife and I heard a popping sound and then a sound of an electrical short. We went into the kitchen to investigate and approximately 15 seconds after the initial noise, another sound of an electrical short and the light bulb had file coming out of the side and darkened the light fixture. I turned the light switch off and the fire went out. There was a significant amount of smoke and visible flame and the house smells like burning/burnt electronics.

The product, in the report: 13-Watt (75W) BR30 Soft White (2700K) Outdoor Flood Light ENERGY STAR BR30 uses 13-watts for 750 lumens of brightness Medium base, most frequently used for track lighting, recessed canned lighting and flood lights Designed for indoor or enclosed use only 2700K color temperature, soft white light ENERGY STAR® qualified
We purchased about 16 of these lights for use throughout the house. They have been used for approximately 3-6 months depending upon the bulb (they were purchased at different times). The reason for this report is we had a failure and it was a very bad failure. The light caused sparks, smoke, and some damage though in this particular case the damage was minor.
The light was located in a recessed can in the bathroom. The bathroom is divided into two areas: one room with a door holds the toilet and shower (which I would consider a damp room) and the other room has just the vanity (which I would consider a non-damp room). The light was located in the vanity/non-damp room. On the basic single-pole switch, there are 3 recessed light (which includes the failed bulb) and the mirror light. On the circuit, there is only ceiling lights/fans. It does not combine with any wall outlets, etc. Of the 3 recessed lights on the circuit, only the one failed. The others did not flicker or dim during the event.
The actual failure event occurred in the morning. The lights to the room had been on for approximately 30 minutes. I was near the vanity mirror getting ready for work, so I witnessed everything. First it made an audible buzzing noise quickly followed by a shower of sparks coming from the bulb. The switch was turned off and the bulb emitted smoke for a few moments. After it cooled down (it felt very hot - much different than the other bulbs), I removed it from the fixture. The sparks had caused some charring to the plastic trim. I tried another bulb, of the same type, in the fixture and it worked as a bulb is expected.
The bulb has a burn mark on its side. In the center of the burn mark there is a hole where you can see inside the bulb now.

The product, in the report: Cool White Dimmable Outdoor Candelabra type LED Lamp
I purchased seven Utilitech Pro #0171150 Outdoor LED Lamps to place in my outdoor lights on 8/22/11. Recently I wanted to remove the bulbs to clean them and to clean the lamp fixtures. Although they were screwed securely into the lamps, they were not overtightened. Upon unscrewing the 7 seven bulbs, 2 of the bulbs broke in my hands. They broke while I was unscrewing them from the sockets. The glass seemed very brittle. I was not cut, but I could see someone else being badly cut. I wrote the company and received no response. I emailed Lowes (where they were purchased) and received an email back stating that the local store would be contacting me. That was about a month ago and I have heard nothing. I reason for reporting this would be to prevent others from being cut by the breaking glass.

The product, in the report: Utilitech 100 watt Compact fluorescent Light bulb Dimmable
I purchased a Utilitech 100-Watt Equivalent Soft White Compact Fluorescent Light Bulb ENERGY COMPLIANT from Lowe’s Hardware on the Internet Model#LBP23TDM Store Item #340445 on Sep 09, 2011. After a week the bulb began flickering badly and kept turning off. I had to loosen the bulb by partly unscrewing and tightening it back up. About a week later I smelled an electrical odor and a smoky haze. The bulb then went out and the odor disipated. I removed the bulb and put it away. I contacted Lowe’s about returning it and they have not bothered to reply. This bulb was highly recommended by a independent Consumer Magazine.
