I was using a Sylvania twist CFL light bulb in a floor lamp. The light bulb base started to char, melt, smoke and create a horrible odor. I turned off the lamp, unplugged it, turned on an exhaust fan. Believe this was a fire hazard. Not sure about the off gassing- plastic fume? mercury?

No one hurt Made Feb 5, 2023, sent to the company Feb 16, 2023, published by CPSC Mar 3, 2023
The product, in the report: Sylvania compact fluorescent bulb 13W 120V 60Hz 175mA HG CF13EL/MINITWIST3000K Y526-1 E137657 7B11 Made in China
The light in the room flickered. Maybe a minute later I noticed a smell similar to burning plastic. I looked around for the source and noticed a stream of smoke rising from behind the lamp shade of the floor lamp. This lamp has a 3-way socket, and three additional sockets for 60 watt or below bulbs. For years we’ve used 13W compact fluorescent bulbs in the lamp. I quickly unplugged the lamp, and carefully removed the smoking bulb. It is a Sylvania 13EL/Minitwist 3000K. At the base of the squiggly bulb where one of the glass legs go into the base that’s above the metal part that contacts the lamp socket, there are brown and black burn marks.

The product, in the report: SYLVANIA Light bulbs Model HALOGEN DL SW 620 lumens 43w/120v
For the last 2-3 yrs, I have purchased all of my light bulbs at the Dollar Tree Store near my house at [REDACTED] Ph # [REDACTED]. I have also bought some bulbs at a Dollar Tree Store in White Marsh MD back in 12/2017.
Last month one of lights in the bathroom vanity exploded. I had just turned the switch on, when it happened, big explosion, it all disintegrated when it exploded. I had to shut down electricity, and use a pair of pliers to unscrew the remaining metal base. I thought it was an isolated, freak accident, and I moved on.
Today the same thing happened on the other bathroom light, when my son turned the lights on, except his time, it didn’t disintegrated,it exploded, but I was able to unscrew it and remove a parts of broken light bulb, you can still read the letters on top of the bulb which reads HALOGEN DL SW 620 LUMENS 43w/120v USA. I had bought a package of the bulbs, I believe hey were on a Sylvania package, red & white packaging.
This is a serious defect and a danger to consumers, when they explode the fragments fly everywhere, they can land on someone’s eyes, face, etc. I still has the light bulb’s remains. I am letting you know so you can recall these bulbs. I have the bulb’s remains and can also take a picture.
Please contact me and let me know if you need anything else. Thanks. [REDACTED]

The product, in the report: LED bulb
Was at home and an LED bulb that has been in a lamp in my home suddenly started making a high pitch whistling noise. I then noticed flames in the lamp shade coming from the opposite side of where the lamp switch was. I then saw thick acrid smoke (dark gray in color) coming from the lamp. I reached up and tried to switch the lamp off, but that did nothing. I then remembered that the outlet into which it was plugged was a switch outlet so I turned the light off and then grabbed the lamp and held the lamp out the window until the flames extinguished.

The product, in the report: Fluorescent light bulb made by Sylvania, 13W 120V 60Hz 190mA CF13EL/MINITWIST
We have been using fluorescent light bulbs for many years. We have 8 bulbs in our main bathroom. Of the 8 bulbs, 6 are made by GE and 2 are made by Sylvania. Couple of days ago I noticed one of the Sylvania stopped working. Then today, the other Sylvania bulb flickered a few times about 1 minute after I turned them on. Then it went out. But soon I smelled something burning and noticed that the light bulb that just went out was burning. I immediately turned off the electricity. But the light bulb had already burned out at one of the two stems. The bulb is about 3 inch below the ceiling, which has been darkened by the burn. Room was filled with very bad strong odor.

The product, in the report: 43 W halogen light bulb soft white A19
My daughter & I were working in our kitchen (our dogs were in the room too) on New Year’s Day when a light bulb in an overhead fixture exploded for no apparent reason. The bulb in question was a recent replacement.. Sylvania 60 W replacement 43W A19 halogen bulbs, extra soft white, Barcode 0 46135 17039 3.
This bulb was new. Put in fixture quite recently. The light had been turned on for an hour or so.
Through sheer good luck we humans were not facing the direction of the bulb when it burst, throwing shards of glass & whatever else is in halogen bulbs ago over the kitchen. There were pieces & minute bits of glass on the dogs.
We were stunned. We could have been seriously injured.
I was further stunned to discover that the company, Sylvania, is well aware that this can happen. In fine print on the end of the box there are warnings in 3 languages about these hazardous bulbs. This is NOT a prominent warning. It’s on the part of the box the package stands on. There are other warnings on the side where the bar code is. A reasonable average person like myself could easily assume ( as I did) that the risks were addressed in this caution area . Based on packaging these appear to be simple, safe, routine replacements for standard bulbs . The same is true of the marketing in the store. Today’s experience shows this simply is not the case..
Research on the Internet & contact with the home improvement store I purchased it from revealed that there is spotty awareness of the risks and special precautions to take installing this type of bulb.
Bottom line: these are unpredictability dangerous products and consumers are inadequately warned about them. I am furious that a corporation can knowingly get away with pushing this dangerous porridge onto an unsuspecting public .

The product, in the report: 48 inch 32 watt t8 10 pack LED Lamps UPC 4613575126 17 t8/l48/fg/841/sub/g6/cvp
I purchase Sylvania LED 48 inch fluorescent replacement that cause the ballast to catch fire in my Home within 5 minutes of installation.
I purchase these lights from Menards, Jeffersonville, Indiana upon the recommendation of the stores salesman. He told my Wife and Brother and My self that they are direct replacements and would work in the existing fixtures.
I reported the incident to the Jeffersonville, Indiana and the Clarksville, Indiana Fire Department prior to returning to the Menards store. The fire department told me to take the problem to someone else. They had no concern for the fire hazard
At the Menards Store, the 3 store managers insisted the problem was not their concern and I should contact the Sylvania if I was not satisfied with the LED Lamps and filed a complaint for me. They told me it will take 6-8 weeks for a reply. They refunded my money and re placed the products back on the display for resale.
The cost of damages to my property is in excess of $400.00 to repair and replace the damaged fixtures.

The product, in the report: osram led bulb a19 e26 8.5 watt 5000k 800 lumens lamp# e327386 120vac 77ma possible date code yx1413
purchased a osram 8.5 watt a19 e26 led bulb from lowes. bulb is installed in a ceiling fixture which is enclosed but can handle 200 watts. within 3 months, this led bulb started cycling on/off, this bulb is in a bedroom and is not on for a long time. I pulled the bulb and it was so hot it took a good 15>20 minutes to be able to hold it. It ran hotter than my soldering iron. there is no way this bulb is energy friendly. I called osram and their solution was to send a replacement, not concern that the product poses a risk. warrant dept. says it can not be in an enclosed fixture and uses this statement to skirt legal issues. the problem is that 99% of these bulbs will be used in an enclosed fixture, how many houses do you know that has only open fixture. the problem is that in the same fixture I run cfl with no issue and a [REDACTED] brand led of the same wattage with no issues. this bulb must have a design defect like the other bulbs they voluntarily recalled.

My son came home from studying and went to his bedroom turned his ceiling fan light on and went to shower. He came out and went to his bedroom and it was filled with smoke and he yelled for me (dad) and when I got in there he looked up and said one of the light bulbs is missing. That is when we looked on the bed and found the light bulb. The metal end cap was still in the light fixture but the plastic end of the bulb was melted. When I got to work Monday morning I typed in the bulb info and found out there was a recall for the same problem. The only damage was the comforter and sheets on his bed has burnt holes and the light socket in the fan light is discolored.

The product, in the report: Halogen PAR20 Bulb
I use Sylvania Capsylite par20 50watt bulbs in a track lighting fixture to illuminate my office. A few months ago, the glass lens from one of them fell off the bulb and onto the desk. Fortunately it landed on some paperwork, otherwise the desk would have been damaged. Last night that same bulb burned out. About an hour afterwards the actual halogen bulb inside it exploded with a loud bang. At first I couldn’t figure out what had happened until I noticed glass shards on the floor, desk, computer screen, keyboard and in my hair. Fortunately I wasn’t injured but feel I could just have easily been blinded had the glass entered my eyes.

SYLVANIA Compact Flourescent Bulb, Model Y529-1, 13 watt, 120 Volt NC, 60 Hertz.
On March 30,2013 at 0500 hours, EST, the aforementioned SYLVANIA compact flourescent bulb, which had been inserted into a chandelier type lighting fixture for some time, was witnessed to suddenly emit sparks, smoke and brief flame. The observants immediately turned off the light fixture, allowed the bulb to cool and removed it. Photos were taken of the defective bulb.
According to Safeproducts.gov, similar model compact flourescent bulbs are experiencing the same defective hazards as noted by numerous complaints.
However, SYLVANIA continues to repond to these safety concerns in a defensive litigious fashion as follows listed in the follwing link. There appears to be a design and or manufacturing defect with this product that could easily, if not already reported, present a danger to life or property loss via result fire as a direct result of this product’s defective design.

The product, in the report: Next to the UL symbol there is a code. 39PR E327386 Sylvania LED light bulb
An LED light bulb manufactured by Sylvania for consumer use in lamps smoked, charred, and exploded while in use in a conventional stand-up lamp. The base of the bulb was charred, and the plastic housing for the bulb dome shattered into several tiny pieces. The bulb had been used without incident in a 5-ft tall stand-up lamp for approximately 18 months. No fire or injuries resulted with this incident, which occurred in the nursery of a two-story colonial home. The lamp was the only device plugged into a conventional outlet in a house built in 1968, not known to have experienced any unusual electrical issues. We have been residents since 2/2011. Assume the bulb was incorrectly matched to the lamp, but I assumed these were safely interchangeable with conventional and/or CFL bulbs. Concerned that others use LED bulbs without concern, when there may be a problem.

The product, in the report: Sylvania Fluorescent dimmable flood light
On the night of Saturday, September 08, 2012, I was awaken by a loud explosion in the computer room where the lights were still on. The outer shell of the fluorescent lamp in the ceiling fan-light fixture blew up and not only it blew up itself, but it exploded the light fixture on the fan. The light fixture on the fan is a standard glass shell, shaped like an open-ended cone, and the light bulb can be installed right through the opening, without needing to remove the fixture. The explosion sent thousands of tiny pieces around, along with about a dozen medium size pieces, and a few large size pieces. The largest projectile was about 5’x2’ with ragged edges. The force of the explosion was such that projectiles went as far as 5 yards, hitting the wall in the hallway, and reflecting in the opposite direction, so one could see the glass in the hallway, before you even turned the corner into the room. The glass was all over the couch, carpet, bookstacks, and my husband and I spent two hours cleaning it up late into the night. Explosion was so powerful that when we lifted the couch pillows, there was glass underneath. When we moved the couch to clean under it, we found the pieces of glass on the wall behind it.
It was the matter of big luck that no one was injured. At the time of explosion, I usually read on the couch, and it was an exception that I went to bed early that day. This product needs to be taken off the market immediately, because it presents a danger of laceration and puncture.
On Monday, September 10, 2012, I contacted the manufacturer, the Osram Sylvania company. My report was recorded and my information taken. I was told that someone from the company will contact me within 48 hours. Nobody ever called or contacted me in any other way, which makes me suspect that the company is aware of the problems with its light bulbs. At a minimum, the company has no interest to ensure their product safety by properly investigating the report.

The product, in the report: screwy light bulb
I have a lamp on an end table that is on a timer and comes on prior to me coming home after work. When I came home a couple of weeks ago, I noticed that the litght was not on and assumed it had burned out. But as I came into the kitchen (which is open to my den) I noticed something on the counter. When I walked over to it, I noticed it was the top of the screwy 3 wayk Chinese light bulb I had in my lamp. When I tgo another light on, I noticed glass as gar as 6 feet away from the lamp, small pieces. the end table was covered and it was covering about half of 2 couches that we sit on most of the time. I had to vacuum it all up. The end table also had a white powder all over it that I could not vacuum, but had to get a damp cloth and wipe it up. I am glad no was sittig on the couches when this bulb, as I understand, emploded because it sure would have hurt them..I had another of these bulbs do the same thing when they first came out, but I did not think too much of it then, I guess. I have grandchildren and if they had been there I’m sure it would have gotten in their eyes. I still have the base to the bulb and the numbers on are E137657 3G01 FCC ID: KQP09. I was sitting in my kitchen at the table on Sunday afternoon, the light overhead was on. I started smelling a terrible burning odor.Looked up and smoke was pouring from my overhead light. I switched it off quickly. after a little while it quit smoking, It cooled of and I removed the bulb. the bulb was a Sylvania 40W CF40EL/TWIST It is one of the new energy type bulbs that if I understand correctly we will be forced to purchase instead of the incadesent type. I don’t know what would have happened if I had not been sitting there. I will not trust these bulbs in the future and am in the process of changing all the bulbs I had put this type in back to the old style. I feel my house may have burned if I had not been there sitting near by and caught what happened. This bulb has been in service for over a year. So it is not new but advertized that it would last for 5 years or more. Please let people know of the dangers of these bulbs, also I understand you cannot dispose of these in the trash they have to be disposed of as flourescent by special means. An the fumes that were in my house are probably toxic and ther should also be warnings of that too,

I turned off my bedroom light and the Sylvania CF13EL/MINITWIST 3000K blinked as thought it was blown out. I walked out of the room to get another bulb and when i returned to the room i smelled something burning and looked up at the ceiling and the bulb was glowing an orange color and fire was shooting out on one side of the tube that is connected to the base of the bulb. I unscrewed the base of the bulb with a towel (very hot). I brought it outside to let it cool down and blew out the fire. It was like blowing out a small Roman candle. No doubt in my mind that would have started a bad fire in no time because the Christmas tree was near by. This is the second time this has happened to me with the exact same type bulb at different physical locations.

While the light bulb was on it began smoking and then I saw a very small flame came out of the base where the pig tail goes into the base . The flame went out when I turned off the light at the light switch. This was a Sylvania low energy light bulb with the pig tail design. As a matter of fact this is the third Sylvania low energy light bulb with the pig tail design that, while it was on, began smoking and a very small flame came out of the base where the pig tail goes into the base within the last year and a half.
If you need the light bulb I will save it until I hear back from you if you would need it or not!

The product, in the report: Sylvania 23W 120V CFL Bulb - CF23EL/MINI TWIST/2, Made in China. Date code 0529110. Also stamped 288J
CFL light bulb installed in standard tabletop lamp with lampshade, with CFL vertical, base down. Light had been on for about 2 hours and suddenly went off without warning or noise. Stayed off for about 30 seconds, then came back on for about 1 - 2 minutes. Flickered and went off again, then came back on. Slight odor noticed. Looked into lamp from above and had strong odor of burnt plastic / hot material. Turned light off. Saw that base of CFL bulb was scorched and melted, base of CFL tube was burnt, very stong smell. Unplugged lamp and took whole unit outside because I thought it was going to catch fire. Opened doors and windows in house to clear the air. Allowed light to cool fo 30 min before removing CFL bulb and taking photos.
Later that evening both my wife and I had severe headaches from the noxious fumes from the burnt plastics / electronics or other components of the light bulb. Concerned about the long term health impact of these fumes and the apparently unsafe nature of these products.

The product, in the report: 13 watts 120 volts mini twist
The fluorescent bulb was placed in an open vanity light for approximately 11 to 12 month total .The light fixture was put in the on position estimating 10-15 minutes when suddenly she smelled smoke and saw smoking coming. While the light bulb was smoking she noticed that the light bulb was still operating.
The light fixture was powered in the off position and the bulb was carefully removed (the item used to remove the bulb is unknown) and she noticed that the plastic casing had burned through.
The bulb was placed in a guest bathroom that is rarely used.
She experienced the same hazard with three other fluorescent bulbs manufactured by the same firm that were also placed in various different fixtures in the home. The bulbs were placed in the home all at the same time and the last three incident occurred a week after the original hazard occurred.
She noticed that the casing at the bottom were brown and scorch looking and emitted a burning smell but they did not catch fire. Around the later part of 2010 she returned all the bulbs to the retailer ad received a full refund to purchase another brand of bulbs.
The firm and was contacted the month 7/2010 and asked the consumer to mail the light bulb in so that they could evaluate the light bulb. The consumer decided not to mail the light bulb back because she does not believe they take her concerns seriously. CONSUMER ADDED: Manufacturer provided inadequate packaging to protect the bulb in transit (an envelope!)

Item in question: Sylvania LED8A/DIM/F/830 light bulb
I was sitting in a nearby room, when I heard a few popping sounds. When I went to investigate, I saw a few sparks falling toward my dining room table. I looked into the light fixture above the table, and found the bulb in question smoking. It was also sitting in the light fixture cover, completely separated from its own screw base. To be clear, if the light fixture cover would not have been in place for some reason, the bulb would have fallen onto my dining room table. Smoke was also emitting from the screw base, and the portion of the light fixture that it was screwed into. It smelled strongly of burned electronics--it’s a smell you don’t forget. The 15-Amp circuit breaker was not tripped during the event. I have since removed the lone screw base from the fixture, and put a new bulb in its place. The only apparent damage to the fixture was caused from heat/burning in the area the LED had been screwed into.

While in the bathroom, I could smell something burning. I looked up at the light bar above the mirror to see one of the Sylvania compact fluorescent light bulbs smoking and the plastic housing melting profusely.
I am extremely dissatisfied with this product and am in fear of my house burning down. About a year ago we replaced ALL of our light bulbs in the house with these thinking it’d be great for our house to be more energy efficient. After this incident I’m regretting investing all this money in this “upgrade” - fearing my house is now a fire hazard due to Sylvania’s faulty design.
