The product, in the report: RPT-B-TOTALTUBE-T8-G3-3FT-840-A a 3 foot Type A/B LED T8 bulb of 4000K color.

I am the Maintenance Director at a skilled nursing facility. On January 15th we had a fire in an overbed light fixture in a resident room, thankfully no one was hurt. A 3 foot RomPhos TotalTube T8 G3 had failed at one end resulting in melting plastic. It was believed it may have been due to a mix of a fluorescent and LED bulb causing a power mismatch. However this morning staff reported a burning smell and it was the same type of bulb that was beginning to fail in the same manner. I purchased a 10-pack of RPT-B-TOTALTUBE-T8-G3-3FT-840-A from Zoro.com in September of 2025. The site now indicated that product is discontinued, but I think people should be warned. Thankfully I keep detailed records and have been able to locate all of the other bulbs in use and remove them from use. I have the 2 damaged bulbs and 8 other bulbs from the same package if you want/need them for any reason.
