The product, in the report: Compact Florescent Light Bulb
April 6, 2022 To: [REDACTED] Regarding: U.S. Consumer Product Safety Commission report [REDACTED] Dated: 06-13-2013
In 2013 my wife and I discovered a problem with one of you compact florescent light bulbs. The problem was documented in the report referenced above. Nine years later we discovered a similar problem in a home that our son and daughter-in-law recently purchased in Mesa Arizona. This time with a different type of compact florescent light bulbs made by your company. We were shocked that after nine years nothing had been done to warn consumers of the problem with your bulbs. No recall or warnings had been issued and yet these bulbs are still in use today. In our initial complaint your company made a statement that you (Philips) had requested that we send the product back to your company for examination. That is not a true statement. We were never contacted by Philips and did not receive any request for the bulbs to be returned. Seeing how your company handled our complaint it seemed obvious that we were in a no-win situation and that any attempt on our part to push this issue further was to no avail. We did pursue the issue further, that is until this week. I am sending you pictures of one of the bulbs we removed from Arizona which clearly shows the glass has been melted indicating extreme heat. We sincerely hope that this time someone will actually make an effort to address this problem and hopefully before it results in property loss or injury. We look forward to hearing from you.

No one hurt Made Jun 30, 2021, sent to the company Jul 23, 2021, published by CPSC Aug 6, 2021
The product, in the report: Philips Lighting 16 watts, 4’ LED, Fluorescent, Light Bulb Model T-8
(6/9/2021) The consumer noticed a burning odor in their home and his wife saw smoke coming from shop light fixture. The consumer turned the lamp off. The end of the light fixture was blackened and charred. The end of the light bulb was also blackened and charred. There have never been any problems with this fixture, prior to this incident. The fixture is approximately 5 – 7 years old.
He has not used this manufacturer’s brand of bulbs in this specific fixture prior to this time.
Shortly after the incident, the consumer emailed the manufacturer to report the matter to them.
• His first email from the company’s rep. indicated that the bulb might have been incompatible with the ballast. • The firm’s second email indicated that there may not have been something wrong with the ballast, but with the bulb. • The consumer sent pictures to the firm and their 3rd email to him, indicated that the pictures showed arcing between the lamp socket and the lamp pins. The firm suggested that the bulb was not installed correctly. The lamp contains an internal fuse that should turn the lamp off if there is a problem. The firm indicated that it appears that the bulb functioned properly in this instance. However, the consumer said it did not, because he had to physically turn the lamp off and it did not turn off on it’s own.
The consumer took the fixture down, went to a retailer and purchased a replacement LED fixture and installed it himself. He paid approximately $38.00, for the replacement fixture.
The consumer has the bulb and fixture and can provide it to CPSC if needed. After 30 dats, if CPSC does not want the bulb & fixture, they will be sent to Signify/Philips.
There were no injuries.

The product, in the report: PHILLIPS Indoor/Outdoor PAR16 Flood, 60W Ordering Code: BC60PAR16/EV/FL 120V
Lens of halogen flood lightbulb in kitchen overhead recessed light fixture shattered explosively. Glass shards were hot enough to melt the nylon rug they landed on. Had someone been typically standing at the sink under the fixture, serious injury might have occurred. The bulb remaining in the fixture was extremely hot and, after removing the bulb from the fixture, evidence of scorching on the fixture was evident. Photos of shattered lens and bulb are attached.

The product, in the report: incandescent light bulb, Philips EcoVantage, 1490 lumens, 72 watts, 120 volts, “Mexico,” are printed on the top of the bulb.
An incandescent light bulb exploded, raining hot glass and metal. It burned holes in furniture and the floor, but fortunately did not burn the person reading under this light. It was a Philips EcoVantage 72-watt, 120-volt bulb of 1490 lumens, also marked “Mexico.” There were two such bulbs in the lamp, and I have removed the surviving one. This product should be recalled. One instance is one too many of this sort of thing, I believe.

I replace light bulbs in all recessed light into Philips Marathon CFL R30 and [REDACTED] when I moved into my current home in May 2010. In less than one year after installing those Marathon light bulbs, I had two incidents of glass globe dropping to the ground. When the 2nd incident happened, I immediately emailed Philips Lighting Support asking for help. Other than the initial automatic replay email, Philips never got back to me. Since then, similar incidents have happened few more times. The most recent incident happened two days, that prompted me to search online (again) and found your site.
I still have the automatic reply email from Philips Lighting Support that includes my message to them describing the very dangerous glass dropping incidents.

The product, in the report: Philips 500 Watt Halogen bulb rated for outdoor lighting fixtures
I have had THREE different Philips 500 watt Halogen bulbs FRACTURE and two of them EXPLODED in my outdoor lighting fixtures. I have two of these fixtures that burn from dusk to dawn on the front of my residence showcasing it in light at night. I have used the 500 watt bulbs for many years (the fixtures are rated and designed for these bulbs), from different manufacturers without the issues I have encountered with the Philips bulbs. This is what happened: In early November 2015, I replaced a burned out bulb in one of the fixtures with a Philips 500 watt halogen bulb. Within a few days, it burned out and upon removal was discovered it had fracture marks in the glass on both ends. A different Philips bulb was installed, and again a few days later, it too burned out. Upon removal it was discovered that one end of this bulb was completely black and had fused molten glass and metal to the fixture pin, ruining the lighting fixture. The other end of the bulb had the same fracture marks as the first bulb. I contacted Philips and they requested both bulbs and the fixture, and all were subsequently sent to Shanghai to their testing facility. The finding was as follows: “fixture pin has signs of oxidation that caused high resistance and lamp burnt at the pinch point.” I was told to regularly polish the pins of the fixture to stop this issue. My lighting fixture was not returned, nor was I compensated for the bulbs. While this lighting fixture was being analyzed for months, I purchased a new lighting fixture as a replacement. Another Philips bulb was installed, and within a few weeks it too burned out. Upon removal it too was discovered that it was fractured on both ends and subsequently broke along these fracture points. It was truly a blessing that the bulb did not damage the new fixture. A [REDACTED] bulb was installed and has been burning nightly with no issues whatsoever since November 2015. I can understand that perhaps somehow oxidation might have caused the damage to the older lighting fixture but that does not explain the problem occurring again in a brand new three week old fixture. Philips completely ignores ALL queries regarding the problem in the new fixture and will not address the issue about the problem occurring in the new fixture with me at all. They just talk about oxidation on the old fixture. They keep telling me they consider this issue serious yet months have gone by and the matter has not been addressed fully. I contacted you because I am truly concerned that in the right environment, these fracturing and exploding bulbs could be dangerous to children or pets, or could even possibly start a deadly fire.

The product, in the report: Philips SlimStyle 10.5w LED (UPC 0-4667743314-7), though problem can be reproduced with older [REDACTED] 60w incandescent
I am a consulting Electrical Engineer but this incident happened in my own home.
Screwed replacement light bulb into a desk lamp. Turned on lamp. Short circuit in lamp tripped house circuit breaker. Examination revealed: * Lamp has a 3-way socket * Bulb was not 3-way (which is allowed/accepted practice) * When bulb is firmly screwed in the additional terminal in the socket for three-way operation was contacting the end of the threaded portion of the lamp causing a short. * Both lamp and socket appear to be constructed properly with no obvious mechanical defect/distortion. * It appears that either the socket, the lamp, or both are slightly out of spec mechanically, allowing the contact in the socket (the one that normally contacts the ring on a 3-way bulb) to deflect sufficiently to contact the bottom edge of the light bulb’s screw base.
* Socket: marked as “[REDACTED]” brand. Appears to match their model [REDACTED] Series ([REDACTED]) see: [REDACTED] Bulb: Philips SlimStyle 10.5w LED (UPC 0-4667743314-7), though problem can be reproduced with older [REDACTED] 60w incandescent * Reported problem to [REDACTED] (via web) and Philips (via phone).

The product, in the report: A Philips LED Candle Candelabra Base (B11) light bulb (for more information visit: http://www.amazon.com/Philips-427799-3-5B11-2700-E12-Candle/dp/B00BEVRF52/ref=sr_1_3?ie=UTF8&qid=1389751470&sr=8-3&keywords=philips+led+candle+lights)
I purchased 9 Philips LED Candle Candelabra Base (B11) light bulbs for my chandelier to replace my existing incandescent light bulbs that were working fine but that I wanted to save on energy costs with the LED light bulbs. I put in the light bulbs and turned on my chandelier and all of the light bulbs turn on but one which is flickering and smoking. I immediately turn off the chandelier and the light bulb is now glued to the socket of the chandelier. I managed to clean out the socket but it is completely toast requiring me to have a repair done. Hopefully you can look into the matter.

The product, in the report: Philips 75w indoor/outdoor PAR30L Flood Model #816255
I have multiple recessed lights(cans) in my kitchen. They have Philips 75W indoor/outdoor PAR 30L Flood bulbs in them (model#816255). Over the last year or so, 3 of the bulbs have failed. When they fail, the glass lens falls off the bulb, crashes to the floor and shatters into thousands of pieces all over my kitchen. Not only is the shattered glass a safety problem, but if one was unlucky enough to be hit on the head it could result in serious injury.

Philips 12.5 watt LED light bulb becomes so hot that you can’t touch it without getting burned. Note: I received an email that some of these light bulbs were being recalled by Philips. When I went to remove the bulbs from the fixture, to check the serial number, the bulb was so hot that it couldn’t be touched without getting burned. I ended up waiting for the bulb to cool a bit and using neoprene pot-holder gloves to protect my hands.

I installed a Philips LED light bulb into a table lamp. When I went to turn on the lamp, I received a 120V shock from the lamp. I assumed it must be the power cord causing the problem, so I replaced the lamp’s wiring. That did not fix the issue. I replaced the LED light bulb with the original incandescent lamp, and the lamp works fine.

The product, in the report: Philips Marathon Mini Decorative Twister EL/mdT 15W bulb
A Philips Marathon mini decorative twister EL/mdT 15W began lighting only intermittently. As I began to replace it to see whether it was the bulb or the wiring of the fixture, I found that something on the bulb was extremely hot -- it caused a painful burn on my finger, where I touched it for an instant. for an instant. The replacement bulb functioned, so the problem was not in the fixture. The bulb was not more than a year or so old, and shouldn’t have burned out; but the problem is that the heat generated was painful and conceivably dangerous, and completely unexpected.

I had a Philips Marathon 23W non-dimmable CFL bulb in a ceiling fixture in my basement. I flipped on the lights and walked downstairs to hear a crackling sound. Searching I see that one of the ceiling lights is flickering lightly and realized there was a flame coming out of the side of the bulbs base. I was lucky it wasn’t one of my kids who went down and just started playing. I was able to flip the lights off immediately and just be left with a light fixture with some smoke marks. These bulbs should not be catching on fire.

The product, in the report: the marathon bulbs are currently on recall
I had two phillips marathon bulbs that came apart and broke when they hit the cupboard. I sent the remaining ten bulbs back to phillips for replacement. They sent me nine after having my bulbs for over two months and claiming they never relieved them. They finally sent me nine bulbs not ten or the other two that broke. I do not feel phillips is doing their part in this recall. I think they just hope people forget or lose their fed ex tracking numbers. So I am still out three bulbs and no response from them.

The product, in the report: Compact Flourescent Bulb, Philips Energy Saver 18W Soft White 4 Bulb Value Pack, 4 Medium Base Mini Twister, Ordering code EL/mdT2 18W 6/4
I installed a Philips Energy Saver Mini Twister 18W Soft White CFL bulb in a kitchen ceiling lamp. About 10 seconds after installing it, the light burned out. About 30 seconds to a minute after that, I tried to remove the bulb to replace it, and as I was twisting it off felt an extremely hot point and removed my hand. I believe I was just touching the first few spirals of the tube, not the base, and the hot point seemed to be the lower spiral. I don’t believe I touched the base at all, but can’t be sure. After treating the burn I got from the bulb, several minutes later, I tried again carefully, cautiously just touching the topmost spiral, and succeed in removing the bulb. After removal, I checked the bulb with my finger and found the base and bottom ring of the spiral tube was quite hot, but no longer enough to burn. There was no smell of burned electronics or plastic, and the outside of the bulb appears normal. I was surprised to see a burned out bulb hot enough to burn my finger. I replaced the bulb with another one from the package and it worked fine. It would be useful to either design the bulbs to not fail in this mode or warn consumers of the risk. The box it is from is a 4 Bulb Value Pack, 4 Medium Base Mini Twister, Philips Energy Saver 18W Soft White, Ordering code EL/mdT2 18W 6/4. I have the bulb available if someone wants to analyze the failure. I reported this to Philips Lighting via their Contact web page today, October 2, 2011.

I have several Philips Marathon light bulbs in my house. About a year ago, the outer covering over the florescent part dropped off. Today, another did the same. Apparently, I’m not alone: http://www.saferproducts.gov/ViewIncident/1171552
Philips told me they’d send a check for all the light bulbs I had in my house. They asked that I mail them back and would provide shipping labels, though not boxes or money to cover the packaging.
When I asked if this was a common problem, a known one, they said there had been incidents reported but nothing widespread enough to think a recall should be issued.
