I purchased several cases of PLED-T818W6KV2-AB-F-20P “hybrid” 4’ T8 LED replacement tubes to replace my conventional fluorescent tubes. While many of these tubes failed in short order, one of these tubes actually caught on fire and fell from the light fixture next to my chair.

No one hurt Made Jun 8, 2024, sent to the company Jun 14, 2024, published by CPSC Jul 1, 2024
The product, in the report: 4 feet LED lightbulb replacing standard T8 fluorescent bulbs, to be used with or without ballast bypass. (Type A+B.) Color temperature 5000K.
Installed Parmida 4FT T8 LED (Type A+B) in standard commercial drop ceiling light fixture with electronic ballast. Several bulbs failed shortly with burnt components and melted plastic. It will become a fire hazard so we will proceed to replace them even though the bulbs are in service for less than 2 months.

The product, in the report: Parmida PLED T8 DS18W5K Bulb. We used these bulbs to convert our whole building from [REDACTED] to LED ballast bypass in 2022.
1/25/24 Chrysalis Hotel, nearly had a fire with a replacement LED bulb: Parmida PLED T8 DS18W5K. We used these bulbs to convert our whole building from T8 florescent to LED ballast bypass in 2022. The end where the electronics are on a bulb shorted and melted, dropping burning debris. Fortunately it caused no fire. The rest of the fixture is still working. Standard dual end ballast bypass rewire job, regular ceramic [REDACTED]. The part that melted was within the electronics at the end of the Parmida LED bulb.

This was the second fire incident that has occurred since they replaced their lightbulbs with the LED lightbulbs in this report (a report was filed on 07/27/2023, following the first fire incident CSPC# [REDACTED]). No one hurt Made Jul 27, 2023, sent to the company Aug 1, 2023, published by CPSC Aug 15, 2023
The product, in the report: PARMIDA 20-pack LED T8 Hybrid Type A+B Light Tube 4’ 18W, clear cover
The [REDACTED] Fire Department Fire Investigation Unit responded to a fire in a lighting fixture that occurred at a high school in the city. Maintenance workers reported that they had changed the lightbulbs in the gymnasium to LED lights approximately 2 weeks prior to the fire. The morning of the fire, the staff found a smoke condition in the school upon entering the building. When they got to the gym, they found a light fixture that had caught fire, resulting in the plastic light covering melting and dropping to the gym floor, burning the floor underneath the light fixture. Damage estimates were $8000. The product was purchased on amazon and when I looked through the reviews, I did find one other report of a fire in a light fixture within a short time of installation of these bulbs.
